GENERAL CONSTRUCTION NOTES
1. UNLESS OTHERWISE STATED, ALL FILL AREAS SHALL BE CONSTRUCTED IN LAYERS OF 8" MAXIMUM THICKNESS,
WITH WATER ADDED OR SOIL CONDITIONED TO THE OPTIMUM MOISTURE CONTENT AS DETERMINED BY THE
ENGINEER AND COMPACTED WITH A SHEEP'S FOOT ROLLER TO A COMPACTION EQUAL TO OR GREATER THAN
95% (100% IN THE TOP 2' OF THE SUB GRADE BELOW ROADWAYS AND PARKING LOTS) OF THE DENSITY
OBTAINED BY COMPACTING A SAMPLE OF THE MATERIAL IN ACCORDANCE WITH THE STANDARD PROCTOR
METHOD OF MOISTURE -DENSITY RELATIONSHIP TEST, ASTM D698 OR AASHTO-99 UNLESS SPECIFIED IN OTHER
SPECIFICATIONS.
2. ENTIRE AREA TO BE GRADED SHALL BE CLEARED AND GRUBBED. NO FILL SHALL BE PLACED ON ANY AREA
NOT CLEARED AND GRUBBED.
3. ALL SOIL EROSION CONTROL MEASURES REQUIRED BY THE GRADING PLAN SHALL BE PERFORMED PRIOR TO
GRADING, CLEARING OR GRUBBING. ALL EROSION CONTROL DEVICES SUCH AS SILT FENCES, ETC., SHALL BE
MAINTAINED IN WORKABLE CONDITION FOR THE LIFE OF THE PROJECT AND SHALL BE REMOVED AT THE
COMPLETION OF THE PROJECT ONLY ON THE ENGINEER'S APPROVAL. PAYMENT SHALL BE CONSIDERED
INCIDENTAL TO CLEARING AND GRUBBING. IF DURING THE LIFE OF THE PROJECT, A STORM CAUSES SOIL
EROSION WHICH CHANGES FINISH GRADES OR CREATES "GULLIES" AND "WASHED AREAS", THESE SHALL BE
REPAIRED AT NO EXTRA COST, AND ALL SILT WASHED OFF OF THE PROJECT SITE ONTO ADJACENT PROPERTY
SHALL BE REMOVED AS DIRECTED BY THE ENGINEER AT NO EXTRA COST. THE CONTRACTOR SHALL ADHERE
TO ANY APPROVED EROSION CONTROL PLANS WHETHER INDICATED IN THE CONSTRUCTION PLANS OR UNDER
SEPARATE COVER.
4. DISPOSABLE MATERIAL
A. CLEARING AND GRUBBING WASTES SHALL BE REMOVED FROM THE SITE AND PROPERLY DISPOSED OF BY
THE CONTRACTOR AT HIS EXPENSE, UNLESS SPECIFIED OTHERWISE. TOPSOIL MAY BE PLACED UP TO 6"
IN DEPTH ON SLOPES AND OPEN AREAS IF AVAILABLE WITH PRIOR APPROVAL BY ENGINEER.
B. SOLID WASTES TO BE REMOVED SUCH AS SIDEWALKS CURBS PAVEMENT ETC., MAY BE PLACED IN
SPECIFIC DISPOSAL AREAS DELINEATED ON THE PLANS OR REMOVED FROM THE SITE AS REQUIRED BY
THE SPECIFICATIONS. THIS MATERIAL SHALL HAVE A MINIMUM COVER OF 2'. THE CONTRACTOR SHALL
MAINTAIN SPECIFIED COMPACTION REQUIREMENTS IN THESE AREAS. WHEN DISPOSAL SITES ARE NOT
PROVIDED, THE CONTRACTOR SHALL REMOVE THIS WASTE FROM THE SITE AND PROPERLY DISPOSE OF IT
AT HIS EXPENSE.
C. ABANDONED UTILITIES SUCH AS CULVERTS, WATER PIPE, HYDRANTS, CASTINGS, PIPE APPURTENANCES,
UTILITY POLES, ETC., SHALL BE THE PROPERTY OF THE SPECIFIC UTILITY AGENCY, OR COMPANY HAVING
JURISDICTION. BEFORE THE CONTRACTOR CAN REMOVE, DESTROY, SALVAGE, REUSE, SELL OR STORE FOR
HIS OWN USE ANY ABANDONED UTILITY, HE MUST PRESENT TO THE OWNER WRITTEN PERMISSION FROM
THE UTILITY INVOLVED.
D. ON SITE BURNING IS PROHIBITED.
5. THE CONTRACTOR IS RESPONSIBLE FOR THE COORDINATION OF ADJUSTMENT OF ALL UTILITY SURFACE
ACCESSES WHETHER HE PERFORMS THE WORK OR A UTILITY COMPANY PERFORMS THE WORK.
6. THE CONTRACTOR SHALL CONTROL ALL "DUST" BY PERIODIC WATERING AND SHALL PROVIDE ACCESS AT ALL
TIMES FOR PROPERTY OWNERS WITHIN THE PROJECT AREA AND FOR EMERGENCY VEHICLES. ALL OPEN DITCHES
AND HAZARDOUS AREAS SHALL BE CLEARLY MARKED IN ACCORDANCE WITH THE SPECIFICATIONS.
7. ALL AREAS WHERE THERE IS EXPOSED DIRT SHALL BE SEEDED, FERTILIZED AND MULCHED ACCORDING TO THE
SPECIFICATIONS. THE FINISHED SURFACE SHALL BE TO GRADE AND SMOOTH, FREE OF ALL ROCKS LARGER
THAN 3", EQUIPMENT TRACKS, DIRT CLODS, BUMPS, RIDGES AND GOUGES PRIOR TO SEEDING; THE SURFACE
SHALL BE LOOSENED TO A DEPTH OF t4"-6" TO ACCEPT SEED. THE CONTRACTOR SHALL NOT PROCEED WITH
SEEDING OPERATIONS WITHOUT FIRST OBTAINING THE ENGINEER'S APPROVAL OF THE GRADED SURFACE. ALL
SEEDING SHALL BE PERFORMED BY A MECHANICAL "HYDRO -SEEDER". HAND SEEDING SHALL BE AUTHORIZED
ON AN AREA BY AREA APPROVAL BY THE ENGINEER. DISTURBED AREAS SHOULD THEN BE MATTED WITH
SHORT-TERM PHOTODEGRADABLE EROSION CONTROL MATTING.
8. WHERE SPECIFIED, STORM DRAIN PIPE SHALL BE REINFORCED CONCRETE PIPE (RCP) CONFORMING TO AASHTO
M-170, AS CONTAINED IN NCDOT STANDARD SPECIFICATION 1032-9 FOR WALL TYPE.
WHERE SPECIFIED, ALL STORM DRAIN PIPE SHALL BE HIGH DENSITY POLYETHYLENE (HDPE). SMOOTH WALL
INTERIOR, WITH WATER TIGHT JOINTS, BACKFILLED WITH # 57 WASHED STONE UP TO MIN. 6" OVER THE TOP
OF THE PIPE. HDPE PIPE USED FOR STORM DRAINAGE DETENTION SYSTEMS SHALL BE ADS N12 WT
OR APPROVED EQUAL.
9. CONTRACTOR SHALL VERIFY ALL ELEVATIONS BEFORE INSTALLATION OF FACILITIES.
10. CATCH BASINS CAST -IN-PLACE SHALL CONFORM TO THE REQUIREMENTS OF NCDOT STANDARD SPECIFICATIONS
FOR ROADS AND STRUCTURES (LATEST EDITION) ARTICLES 840-1 THROUGH 840-3. CURB INLET CATCH BASIN
SHALL CONFORM TO NCDOT STANDARD DETAILS 840.02 THROUGH 840.04. DROP INLETS SHALL CONFORM TO
STANDARD DETAIL 840.14. JUNCTION BOXES SHALL CONFORM TO STANDARD DETAIL 840.31.
11. CURB INLET FRAME, GRATE AND HOOD SHALL BE NEENAH R -3233D, PRODUCTS BY DEWEY BROS., U.S.
FOUNDRY OR EQUAL. DROP INLET FRAME AND GRATE SHALL BE NEENAH R -3339A OR EQUAL. FIELD INLET
COVER SHALL CONFORM TO NCDOT STANDARD DETAIL 840.04, OPENING FACING UPSTREAM.
12. CONCRETE AND MASONRY SHALL MEET THE REQUIREMENTS OF APPROPRIATE SECTION OF NCDOT STANDARD
SPECIFICATIONS FOR ROAD AND STRUCTURES (LATEST EDITION). CONCRETE SHALL BE CLASS A OR B, 4000
PSI MINIMUM, MEETING THE REQUIREMENTS OF SECTION 1000, CONSTRUCTED IN ACCORDANCE WITH SECTION
825. MASONRY SHALL MEET THE REQUIREMENTS OF SECTION 1040, CONSTRUCTED IN ACCORDANCE WITH
SECTION 830 AND/OR 834.
13. TOPS OF PROPOSED FRAMES AND GRATES SHALL BE FLUSH WITH FINISHED GRADE.
14 NO BASE COURSE SHALL BE PLACED ON MUCK, PIPE, CLAY, ORGANIC MATTER, OR OTHER UNSUITABLE
MATTER, AND A MINIMUM COMPACTION RATE OF SUBGRADE PRIOR TO PAVING SHALL NOT BE LESS THAN 95%
BY STANDARD PROCTOR METHOD AND CERTIFIED BY A PROFESSIONAL ENGINEER.
15. ALL RETAINING WALLS SHOWN WILL BE DESIGNED BY A LICENSED NORTH CAROLINA PROFESSIONAL ENGINEER
PRIOR TO PERMITTING AND CONSTRUCTION OF THE WALL.
NOTES: STATEMENT OF PERMIT AND CONTRACTOR
RESPONSIBILITIES
1. PROJECT SITE CONTAINS JURISDICTIOANL WATERS OF THE U.S (WOTUS).
2. PROJECT INVOLVES IMPACTS TO WOTUS IN SEVERAL LOCATIONS.
3. PROPOSED IMPACTS TO WOTUS ARE REGULATED BY THE CLEAN WATER ACT
SECTIONS 404 AND 401.
4. PERMITS HAVE BEEN OBTAINED BY THE CITY OF XXX INC TO AUTHORIZE THE
PROPOSED IMPACTS TO WOTUS AND ARE SUBJECT TO THE TERMS AND
CONDITIONS OF THE PERMITS.
5. CWA 404/401 PERMIT REQUIREMENTS, TERMS, AND CONDITIONS ARE SUMMARIZED
ON THIS SHEET ALONG WITH VARIOUS ASSOCIATED DETAILS AND SPECIFICATIONS.
6. A COPY OF THE PERMITS HAVE BEEN PROVIDED TO THE CONTRACTOR AT THE
PRE -CONSTRUCTION MEETING. THE CONTRACTOR IS RESPONSIBLE FOR READING
THE PERMITS, PLANS AND SPECIFICATIONS, AND UNDERSTANDING HIS/HER
RESPONSIBILITIES UNDER THE PERMITS, WHICH INCLUDE BUT ARE NOT LIMITED TO:
A) COMPLIANCE WITH ALL PERMIT CONDITIONS RELATING TO CONTRACTED
CONSTRUCTION ACTIVITIES BOTH ON-SITE AND OFF-SITE, IF OFF-SITE AREAS
ARE USED FOR PROJECT RELATED PURPOSES (EG: ACCESS, LAYDOWN, STAGING,
BORROW, WASTE, ETC.)
B) COMMUNICATION OF PERMIT COMPLIANCE RESPONSIBILITIES TO SUBCONTRACTORS
WORKING ON THE PROJECT.
C) COORDINATION WITH PROJECT ENGINEER REGARDING PERMIT COMPLIANCE RELATED
ISSUES AND QUESTIONS.
D) IMMEDIATE REPORTING TO PROJECT ENGINEER OR OWNER OF ANY VIOLATION OR
SUSPECTED VIOLATION OF PERMIT CONDITIONS.
E) A COPY OF ALL PERMITS AND CONDITIONS SHOULD BE MADE AVAILABLE AT THE
PROJECT SITE DURING CONSTRUCTION AND MAINTENANCE ACTIVITIES.

NATIONWIDE PERMIT CONDITIONS
1. AQUATIC LIFE MOVEMENTS - NO ACTIVITY MAY SUBSTANTIALLY DISRUPT THE NECESSARY LIFE
CYCLE MOVEMENTS OF THOSE SPECIES OF AQUATIC LIFE INDIGENOUS TO THE WATERBODY,
INCLUDING THOSE SPECIES THAT NORMALLY MIGRATE THROUGH THE AREA UNLESS THE ACTIVITY'S
PRIMARY PURPOSE IS TO IMPOUND WATER. ALL PERMANENT AND TEMPORARY CROSSINGS OF
WATERBODIES SHALL BE SUITABLY CULVERTED, BRIDGED, OR OTHERWISE DESIGNED AND
CONSTRUCTED TO MAINTAIN LOW FLOWS TO SUSTAIN THE MOVEMENT OF THOSE AQUATIC SPECIES.
2. SUITABLE MATERIAL - NO ACTIVITY MAY USE UNSUITABLE MATERIAL (E.G., TRASH, DEBRIS, CAR
BODIES, ASPHALT, ETC.). MATERIAL USED FOR CONSTRUCTION OR DISCHARGED MUST BE FREE
FROM TOXIC POLLUTANTS IN TOXIC AMOUNTS (SEE SECTION 307 OF THE CLEAN WATER ACT).
3. MANAGEMENT OF WATER FLOWS - TO THE MAXIMUM EXTENT PRACTICABLE, THE
PRE -CONSTRUCTION COURSE, CONDITION, CAPACITY, AND LOCATION OF OPEN WATERS MUST BE
MAINTAINED FOR EACH ACTIVITY, INCLUDING STREAM CHANNELIZATION AND STORM WATER
MANAGEMENT ACTIVITIES, EXCEPT AS PROVIDED BELOW. THE ACTIVITY MUST BE CONSTRUCTED TO
WITHSTAND EXPECTED HIGH FLOWS. THE ACTIVITY MUST NOT RESTRICT OR IMPEDE THE PASSAGE
OF NORMAL OR HIGH FLOWS, UNLESS THE PRIMARY PURPOSE OF THE ACTIVITY IS TO IMPOUND
WATER OR MANAGE HIGH FLOWS. THE ACTIVITY MAY ALTER THE PRE -CONSTRUCTION COURSE,
CONDITION, CAPACITY, AND LOCATION OF OPEN WATERS IF IT BENEFITS THE AQUATIC
ENVIRONMENT (E.G., STREAM RESTORATION OR RELOCATION ACTIVITIES).
4. EQUIPMENT - HEAVY EQUIPMENT WORKING IN WETLANDS OR MUDFLATS MUST BE PLACED ON
MATS, OR OTHER MEASURES MUST BE TAKEN TO MINIMIZE SOIL DISTURBANCE.
5. SOIL EROSION AND SEDIMENT CONTROLS - APPROPRIATE SOIL EROSION AND SEDIMENT CONTROLS
MUST BE USED AND MAINTAINED IN EFFECTIVE OPERATING CONDITION DURING CONSTRUCTION, AND
ALL EXPOSED SOIL AND OTHER FILLS, AS WELL AS ANY WORK BELOW THE ORDINARY HIGH WATER
MARK OR HIGH TIDE LINE, MUST BE PERMANENTLY STABILIZED AT THE EARLIEST PRACTICABLE
DATE. PERMITTEES ARE ENCOURAGED TO PERFORM WORK WITHIN WATERS OF THE UNITED STATES
DURING PERIODS OF LOW -FLOW OR NO -FLOW.
6. REMOVAL OF TEMPORARY FILLS - TEMPORARY FILLS MUST BE REMOVED IN THEIR ENTIRETY AND
THE AFFECTED AREAS RETURNED TO PRE -CONSTRUCTION ELEVATIONS. THE AFFECTED AREAS MUST
BE REVEGETATED, AS APPROPRIATE.
7. PROPER MAINTENANCE - ANY AUTHORIZED STRUCTURE OR FILL SHALL BE PROPERLY MAINTAINED,
INCLUDING MAINTENANCE TO ENSURE PUBLIC SAFETY AND COMPLIANCE WITH APPLICABLE NWP
GENERAL CONDITIONS, AS WELL AS ANY ACTIVITY -SPECIFIC CONDITIONS ADDED BY THE DISTRICT
ENGINEER TO AN NWP AUTHORIZATION.
8. DISCOVERY OF PREVIOUSLY UNKNOWN REMAINS AND ARTIFACTS - IF YOU DISCOVER ANY
PREVIOUSLY UNKNOWN HISTORIC, CULTURAL OR ARCHEOLOGICAL REMAINS AND ARTIFACTS WHILE
ACCOMPLISHING THE ACTIVITY AUTHORIZED BY THIS PERMIT, YOU MUST IMMEDIATELY NOTIFY THE
DISTRICT ENGINEER OF WHAT YOU HAVE FOUND, AND TO THE MAXIMUM EXTENT PRACTICABLE,
AVOID CONSTRUCTION ACTIVITIES THAT MAY AFFECT THE REMAINS AND ARTIFACTS UNTIL THE
REQUIRED COORDINATION HAS BEEN COMPLETED. THE DISTRICT ENGINEER WILL INITIATE THE
FEDERAL, TRIBAL AND STATE COORDINATION REQUIRED TO DETERMINE IF THE ITEMS OR REMAINS
WARRANT A RECOVERY EFFORT OR IF THE SITE IS ELIGIBLE FOR LISTING IN THE NATIONAL
REGISTER OF HISTORIC PLACES.
9. RESTRICTION ON USE OF LIVE CONCRETE - FOR ALL NWPS WHICH ALLOW THE USE OF CONCRETE
AS A BUILDING MATERIAL, LIVE OR FRESH CONCRETE, INCLUDING BAGS OF UNCURED CONCRETE,
MAY NOT COME INTO CONTACT WITH THE WATER IN OR ENTERING INTO WATERS OF THE US. WATER
INSIDE COFFER DAMS OR CASINGS THAT HAS BEEN IN CONTACT WITH WET CONCRETE SHALL ONLY
BE RETURNED TO WATERS OF THE US WHEN IT IS NO LONGER POSES A THREAT TO AQUATIC
ORGANISMS.
10. REQUIREMENTS FOR USING RIPRAP FOR BANK STABILIZATION - FOR ALL NWPS THAT ALLOW FOR
THE USE OF RIPRAP MATERIAL FOR BANK STABILIZATION, THE FOLLOWING MEASURES SHALL BE
APPLIED:
10.1. FILTER CLOTH MUST BE PLACED UNDERNEATH THE RIPRAP AS AN ADDITIONAL REQUIREMENT
OF ITS USE IN NORTH CAROLINA WATERS.
10.2. THE PLACEMENT OF RIPRAP SHALL BE LIMITED TO THE AREAS DEPICTED ON SUBMITTED WORK
PLAN DRAWINGS.
10.3. THE RIPRAP MATERIAL SHALL BE CLEAN AND FREE FROM LOOSE DIRT OR ANY POLLUTANT
EXCEPT IN TRACE QUANTITIES THAT WOULD NOT HAVE AN ADVERSE ENVIRONMENTAL EFFECT.
10.4. IT SHALL BE OF A SIZE SUFFICIENT TO PREVENT ITS MOVEMENT FROM THE AUTHORIZED
ALIGNMENT BY NATURAL FORCES UNDER NORMAL CONDITIONS.
10.5. THE RIPRAP MATERIAL SHALL CONSIST OF CLEAN ROCK OR MASONRY MATERIAL SUCH AS,
BUT NOT LIMITED TO, GRANITE, MARL, OR BROKEN CONCRETE.
11. SAFE PASSAGE REQUIREMENTS FOR CULVERT PLACEMENT - FOR ALL NWPS THAT INVOLVE THE
CONSTRUCTION/INSTALLATION OF CULVERTS, MEASURES WILL BE INCLUDED IN THE
CONSTRUCTION/INSTALLATION THAT WILL PROMOTE THE SAFE PASSAGE OF FISH AND OTHER
AQUATIC ORGANISMS. THE DIMENSION, PATTERN, AND PROFILE OF THE STREAM ABOVE AND BELOW
A PIPE OR CULVERT SHOULD NOT BE MODIFIED BY WIDENING THE STREAM CHANNEL OR BY
REDUCING THE DEPTH OF THE STREAM IN CONNECTION WITH THE CONSTRUCTION ACTIVITY. THE
WIDTH, HEIGHT, AND GRADIENT OF A PROPOSED CULVERT SHOULD BE SUCH AS TO PASS THE
AVERAGE HISTORICAL LOW FLOW AND SPRING FLOW WITHOUT ADVERSELY ALTERING FLOW VELOCITY.
SPRING FLOW SHOULD BE DETERMINED FROM GAGE DATA, IF AVAILABLE. IN THE ABSENCE OF SUCH
DATA, BANKFULL FLOW CAN BE USED AS A COMPARABLE LEVEL.
EXISTING CONDITIONS LEGEND
NCDEQ GENERAL CONSTRUCTION CONDITIONS
1. THE CONTRACTOR SHALL MINIMIZE CLEARING WITHIN WETLANDS AND ACROSS STREAMS
TO THE GREATEST EXTENT PRACTICABLE AND SHALL NOT EXCEED 50' WIDE FOR GAS
UTILITY LINES AND 40' WIDE FOR ALL OTHER UTILITY LINES. EXCEPTIONS TO THIS
CONDITION WILL REQUIRE WRITTEN APPROVAL FROM THE PROJECT ENGINEER. FOR
CONSTRUCTION CORRIDORS IN WETLANDS AND ACROSS STREAM CHANNELS, STUMPS
SHALL BE GRUBBED ONLY AS NEEDED TO INSTALL THE UTILITY AND REMAINING STUMPS
SHALL BE CUT OFF AT GRADE LEVEL. THE GENERAL STRIPPING OF TOPSOIL WITHIN
WETLANDS ALONG THE CONSTRUCTION CORRIDOR IS NOT PERMITTED.
2. PERMANENT MAINTAINED ACCESS CORRIDORS IN WETLANDS AND ACROSS STREAM
CHANNELS SHALL BE RESTRICTED TO THE MINIMUM WIDTH

11. APPLICATION OF FERTILIZER TO ESTABLISH PLANTED/SEEDED VEGETATION WITHIN

ACTIVITIES MANUAL, SUCH AS SANDBAGS, ROCK BERMS, COFFERDAMS, AND OTHER
DIVERSION STRUCTURES SHALL BE USED TO MINIMIZE EXCAVATION IN FLOWING WATER.
EXCEPTIONS TO THIS CONDITION REQUIRE WRITTEN APPROVAL FROM THE PROJECT
ENGINEER.
10. CULVERTS SHALL BE INSTALLED IN SUCH A MANNER THAT THE ORIGINAL STREAM
PROFILES ARE NOT ALTERED AND ALLOW FOR AQUATIC LIFE MOVEMENT DURING LOW
FLOWS. THE DIMENSION PATTERN AND PROFILE OF THE STREAM ABOVE AND BELOW A
PIPE OR CULVERT SHALL NOT BE MODIFIED BY WIDENING THE STREAM CHANNEL OR BY
REDUCING THE DEPTH OF THE STREAM IN CONNECTION WITH THE CONSTRUCTION
ACTIVITY.
11. APPLICATION OF FERTILIZER TO ESTABLISH PLANTED/SEEDED VEGETATION WITHIN
DISTURBED RIPARIAN AREAS SHALL COMPLY WITH ALL OTHER FEDERAL, STATE AND
LOCAL REGULATIONS. FERTILIZER APPLICATION SHALL BE ACCOMPLISHED IN A MANNER
THAT MINIMIZES THE RISK OF CONTACT BETWEEN THE FERTILIZER AND SURFACE WATERS.
12. IF CONCRETE IS USED DURING CONSTRUCTION, THEN ALL NECESSARY MEASURES SHALL
BE TAKEN TO PREVENT DIRECT CONTACT BETWEEN UNCURED OR CURING CONCRETE AND
WATERS OF THE US. WATER THAT INADVERTENTLY CONTACTS UNCURED CONCRETE
SHALL NOT BE DISCHARGED TO WATERS OF THE US.
13. ALL PROPOSED AND APPROVED TEMPORARY FILL AND CULVERTS SHALL BE REMOVED
AND THE IMPACTED AREA RETURNED TO NATURAL CONDITIONS WITHIN 60 CALENDAR
DAYS AFTER THE TEMPORARY IMPACT IS NO LONGER NECESSARY. THE IMPACTED AREAS
SHALL BE RETURNED TO ORIGINAL GRADE, INCLUDING EACH STREAM'S ORIGINAL CROSS
SECTIONAL DIMENSIONS, PLANFORM PATTERN, AND LONGITUDINAL BED PROFILE. ALL

15. ANY RIP -RAP REQUIRED FOR PROPER CULVERT PLACEMENT, STREAM STABILIZATION, OR
RESTORATION OF TEMPORARILY DISTURBED AREAS SHALL BE RESTRICTED TO THE AREA
DIRECTLY IMPACTED BY THE APPROVED CONSTRUCTION ACTIVITY. ALL RIP -RAP SHALL

Note: After the permanent cessation of construction activities, any areas with temporary
ground stabilization shall be converted to permanent ground stabilization as soon as
practicable but in no case longer than 90 calendar days after the last land disturbing

POLYACRYLAMIDES (PAMS) AND FLOCCULANTS
1. Select flocculants that are appropriate for the soils being exposed during
construction, selecting from the NC DWR List of Approved PAMS/Flocculants.
2. Apply flocculants at or before the inlets to Erosion and Sediment Control Measures.
3. Apply flocculants at the concentrations specified in the NC DWR List of Approved
PAMS/Flocculants and in accordance with the manufacturer's instructions.
4. Provide ponding area for containment of treated Stormwater before discharging
offsite.
5. Store flocculants in leak -proof containers that are kept under storm -resistant cover
or surrounded by secondary containment structures.
EQUIPMENT AND VEHICLE MAINTENANCE
1. Maintain vehicles and equipment to prevent discharge of fluids.
2. Provide drip pans under any stored equipment.
3. Identify leaks and repair as soon as feasible, or remove leaking equipment from the
Project.
4. Collect all spent fluids, store in separate containers and properly dispose as
hazardous waste (recycle when possible).
5. Remove leaking vehicles and construction equipment from service until the problem
has been corrected.
6. Bring used fuels, lubricants, coolants, hydraulic fluids and other petroleum products
to a recycling or disposal center that handles these materials.
LITTER, BUILDING MATERIAL AND LAND CLEARING WASTE
1. Never bury or burn waste. Place litter and debris in approved waste containers.
2. Provide a sufficient number and size of waste containers (e.g dumpster, trash
receptacle) on site to contain construction and domestic wastes.
3. Locate waste containers at least 50 feet away from storm drain inlets and surface
waters unless no other alternatives are reasonably available.
4. Locate waste containers on areas that do not receive substantial amounts of runoff
from upland areas and does not drain directly to a storm drain, stream or wetland.
5. Cover waste containers at the end of each workday and before storm events or
provide secondary containment. Repair or replace damaged waste containers.
6. Anchor all lightweight items in waste containers during times of high winds.
7. Empty waste containers as needed to prevent overflow. Clean up immediately if
containers overflow.
8. Dispose waste off-site at an approved disposal facility.
9. On business days, clean up and dispose of waste in designated waste containers.
PAINT AND OTHER LIQUID WASTE
1. Do not dump paint and other liquid waste into storm drains, streams or wetlands.

with properly operating unit.
EARTHEN STOCKPILE MANAGEMENT
1. Show stockpile locations on plans. Locate earthen -material stockpile areas at least
50 feet away from storm drain inlets, sediment basins, perimeter sediment controls
and surface waters unless it can be shown no other alternatives are reasonably
available.
2. Protect stockpile with silt fence installed along toe of slope with a minimum offset of
five feet from the toe of stockpile.
3. Provide stable stone access point when feasible.
4. Stabilize stockpile within the timeframes provided on this sheet and in accordance
with the approved plan and any additional requirements. Soil stabilization is defined
as vegetative, physical or chemical coverage techniques that will restrain accelerated
erosion on disturbed soils for temporary or permanent control needs.

1. Do not discharge concrete or cement slurry from the site.
2. Disposeor recycle of settled hardened concrete residue in accordance with local
and state solid waste regulations and at an approved facility.
3. Manage washout from mortar mixers in accordance with the above item and in
addition place the mixer and associated materials on impervious barrier and within
lot perimeter silt fence.
4. Install temporary concrete washouts per local requirements, where applicable. If an
alternate method or product is to be used, contact your approval authority for
review and approval. If local standard details are not available, use one of the two
types of temporary concrete washouts provided on this detail.
5. Do not use concrete washouts for dewatering or storingdefective curb or sidewalk
sections. Stormwater accumulated within the washout may not be pumped into or
discharged to the storm drain system or receiving surface waters. Liquid waste must
be pumped out and removed from project.
6. Locate washouts at least 50 feet from storm drain inlets and surface waters unless it
can be shown that no other alternatives are reasonably available. At a minimum,
install protection of storm drain inlet(s) closest to the washout which could receive
spills or overflow.
7. Locate washouts in an easily accessible area, on level ground and install a stone
entrance pad in front of the washout. Additional controls may be required by the
approving authority.
8. Install at least one sign directing concrete trucks to the washout within the project
limits. Post signage on the washout itself to identify this location.
9. Remove leavings from the washout when at approximately 75% capacity to limit
overflow events. Replace the tarp, sand bags or other temporary structural
components when no longer functional. When utilizing alternative or proprietary
products, follow manufacturer's instructions.
10. At the completion of the concrete work remove remaining leavings and dispose of

in an approved disposal facility. Fill pit, if applicable and stabilize any disturbance
caused by removal of washout.
HERBICIDES, PESTICIDES AND RODENTICIDES
1. Store and apply herbicides, pesticides and rodenticides in accordance with label
restrictions.
2. Store herbicides, pesticides and rodenticides in their original containers with the
label, which lists directions for use, ingredients and first aid steps in case of
accidental poisoning.
3. Do not store herbicides, pesticides and rodenticides in areas where flooding is
possible or where they may spill or leak into wells, stormwater drains, ground water
or surface water. If a spill occurs, clean area immediately.
4. Do not stockpile these materials onsite.
HAZARDOUS AND TOXIC WASTE
1. Create designated hazardous waste collection areas on-site.
2. Place hazardous waste containers under cover or in secondary containment.
3. Do not store hazardous chemicals, drums or bagged materials directly on the ground.

GENERAL: ALL EROSION CONTROL MEASURES ARE TO BE PERFORMED IN STRICT ACCORDANCE WITH
REQUIREMENTS OF THE NORTH CAROLINA DEPARTMENT OF ENVIRONMENTAL QUALITY (NCDEQ), DIVISION
OF ENERGY, MINERAL, AND LAND RESOURCES (DEMLR), LAND QUALITY SECTION. THE FOLLOWING
CONSTRUCTION SEQUENCE SHALL BE COMPLIED WITH FOR ALL WORK.
1. PRIOR TO BEGINNING WORK ON THE PROJECT THE CONTRACTOR SHALL OBTAIN FROM THE OWNER A

MINERAL, AND LAND RESOURCES (DEMLR), DIVISION OF LAND QUALITY, OR THE LOCAL AUTHORIZED
PROGRAM. THE APPROVAL NOTICE AND CERTIFICATE OF COVERAGE MUST BE AVAILABLE ON-SITE

3. ROLL THE RECP'S DOWN OR HORIZONTALLY ACROSS THE SLOPE. RECP'S WILL UNROLL WITH APPROPRIATE SIDE AGAINST THE SOIL SURFACE.
ALL RECP'S MUST BE SECURELY FASTENED TO SOIL SURFACE BY PLACING AND SPACING STAPLES/STAKES IN APPROPRIATE LOCATIONS PER
MANUFACTURER RECOMMENDATIONS.
4. THE EDGES OF PARALLEL RECP'S MUST BE STAPLED WITH APPROXIMATELY 2"-5" OVERLAP DEPENDING ON RECP'S TYPE.

1. INSTALL ALL EROSION CONTROL MEASURES AS SHOWN ON EROSION
CONTROL PLAN AND IN THIS DETAIL.
2. CONSTRUCT IMPERVIOUS DIKE (RECOMMEND SAND BAGS OR OTHER EASILY
REMOVABLE MATERIAL) ON UPSTREAM SIDE OF EXISTING CHANNEL AND
INSTALL TEMPORARY BYPASS PUMP TO DIVERT WATER AROUND CONSTRUCTION
AREA. ALL MATERIAL MUST BE COMPLETELY REMOVED FOLLOWING
CONSTRUCTION.
3. CONSTRUCT IMPERVIOUS DIKE (RECOMMEND SAND BAGS OR OTHER EASILY

4. DEWATER THE CONSTRUCTION AREA USING ANOTHER TEMPORARY PUMP
AND DISCHARGE WATER INTO A DEWATERING BASIN UNTIL SEDIMENT HAS
SETTLED AND WATER CAN BE DISCHARGED BACK INTO STREAM. DEWATERING
BASIN MUST BE OUTSIDE OF ANY RESTRICTED BUFFER AREAS, USE WATTLES
INSTEAD OF EARTH BERM IF REASONABLE.
5. BEGIN CONSTRUCTION OF CROSS -CULVERT. CONTRACTOR SHALL INSPECT
SYSTEM DAILY BEFORE BEGINNING ANY CONSTRUCTION. NO CONSTRUCTION
SHALL CONTINUE IF SYSTEM IS NOT FULLY OPERATIONAL.
6. ANY WASH MATERIAL REMOVED FROM BYPASS AREA SHOULD BE STONED
OUTSIDE THE FLOOD ZONE OR REGULATED BUFFER AREA UNTIL CONSTRUCTION
IS COMPLETE.
7. UPON COMPLETION OF CULVERT CONSTRUCTION, REMOVE BYPASS SYSTEM
WITHIN (2) TWO CALENDAR DAYS.
8. STABILIZE ALL AREAS DISTURBED DURING CONSTRUCTION. STREAM BED AND
BANKS MUST BE RETURNED TO PRE -CONSTRUCTION CONDITIONS.
9. CONTRACTOR SHALL RECEIVE PRIOR APPROVAL FROM ENGINEER IF
CHOOSING TO USE ANOTHER METHOD IN LIEU OF BYPASS CHANNEL.
