GW -59A Form.

The Cooling Pond Remedial Action System Design Report was submitted to NCDENR on September 27, 1990 and
approved by NCDENR January 7, 1991. The system was installed by PCS Phosphate between July 8, 1991 and
April 9,1992. In 2008 the cut-off wall was extended on the south perimeter of CP -1. This action was taken due to
the location of the active mine pit.
