information. Contact and secure, approval from the DWQ prior to transfer of waste to a site
not covered in the facility's CAWMP.
Operation will be partially or fully depopulated.
*Attach a complete schedule with corresponding animal units and dates for depopulation.
*if animals are to be moved to another permitted facility, provide facility number, lagoon
freeboard levels and herd population for the receiving facility

Irrigation onto existing spray fields will continue as weather and field conditions permit.
Prior to Hurricane Florence, lagoon was in compliance at 41 inches freeboard. Due to heavy rainfall from Hurricane
Florence, DWR was notified of inundation of approx. 36 hours or less. Animal waste'system was monitored daily for signs
of discharge or breach, with none noted. At inspection on 1011/18, freeboard was at 14 inches with no evidence of
structural concern. On 10/29/18 freeboard is at 32 inches. Information stated is as of call in date and was updated on
10/29/18
