Farkas(a-)-ncdenr.gov A review of the Application submitted on May 6, 2019 has determined that the application is not complete. The following information is needed to continue the stormwater review: 1. Please include the stream name and classification for where the roadside ditch ultimately outfalls to on the application (Section IV, 10, Receiving Stream Name). RESPONSE: Stream name and classification were added into Section IV of the revised SMP application. 2. Please include only the area that drains to the BMP, not the entire property area, in the drainage area section of the application (Section IV, 10, Total Drainage Area). Please also recalculate the values for on -site and off -site drainage areas to reflect this change. RESPONSE: Areas draining exclusively to the BMP were added into Section IV of the revised SMP application. Off -site pervious area was also added to Section IV and the stormwater calculations. 3. Please complete a Supplement EZ form for each BMP. The Supplement EZ form can be found here: https://deg.nc.gov/about/divisions/energy-mineral-land- resources/energy- mineral-land-permit-quidance/stormwater-bmp-manual (Section V). RESPONSE: Supplement EZ forms for the proposed wetland and the treatment swa/e were completed and are enclosed. Page 1 of 4 SAIMOS `• EIJC,IIJEERfIJC,, LLC. Response to Review Comments 4. Please provide signed, sealed, and dated design calculations (Section VI, 7). Calculations should include, but are not limited to: a. Calculations showing that the rip -rap at the inlet of the stormwater wetland and lack of rip -rap downstream of M.E.S.-3 are sufficient to prevent erosion during the peak flow from the 10-year storm (As per 15A NCAC 02H .1050(4)). RESPONSE: Drainage system calculation included in Appendix D of the Stormwater Report indicate a maximum "physical" velocity of 5.0 fps in MES-1 and 4.6 fps in MES-3 for the 10-year storm. A grassed swa/e will provide adequate erosion protection on the swa/es at these velocities. However, a rip -rap protection was added at both M.E.S outlets to provide additional protection. b. Calculations showing that the 0.75in/hr storm will pass through the grass swale at a rate of 0.5 fps or less, with a depth of 6 inches or less, in at least 4 minutes (As per 15A NCAC 02H .1061(3)) RESPONSE: Calculations for the grassed swa/e during the 0.75 in/hr storm were added in Appendix D of the Stormwater Report. c. Calculations showing that the 10-year storm will be passed at a non -erosive velocity (As per 15A NCAC 02H .1061(5). RESPONSE: Calculations for the grassed swa/e during the 10-year storm were added in Appendix D of the Stormwater Report. 5. Please delineate the wetlands on the plans or provide a note stating that no wetlands exist. Provide documentation of the qualifications and identity of the individual who made this determination (Section VI, 8, m). RESPONSE: Please see wetland assessment letter from Environmental Services, Inc. included in Appendix E of the Stormwater Report. Also, refer to sheet C2.0 for existing small man-made stormwater basin. 6. Please show the drainage areas for the BMPs on the plans (Section VI, 8, o). RESPONSE: Drainage areas for the BMP are shown on the Post -Development Basin Map (Exhibit PB) included in Appendix A of the Stormwater Report. Basin map and calculations were revised to include an off -site area. 7. Please provide a copy of the current property deed (Section VI, 10). RESPONSE: A copy of the warranty deed is included with this resubmittal package. 8. Please provide official documents with name, title, and position for those listed in Section III of the application (Section VI, 11). RESPONSE: A Power of Attorney for the signing official Kenneth Haynes and NC Business Corporation Officer registration for Richard D. Bishop is included with this resubmittal package. The spot elevations of the grass swale, 823.70 feet on the downstream side and 824.00 feet (Shown incorrectly in the stormwater management report, page 4, and inferred from cross-section A -A on sheet C2.0) imply that the grass swale has a slope of 0.003 ft/ft. This slope is shown as 0.03% (Equal to 0.0003 ft/ft) on the plans. Please revise for consistency. RESPONSE: Grassed swale geometry and slope have been revised. Proposed slope is now 0.04%, plans and calculations were revised accordingly. 10. The stormwater wetland grading shown in the plans does not match the grading shown in the stormwater management report. Please revise for consistency. RESPONSE: Plans and report were revised for consistency. 11. The forebay area appears to be less than 10% of the stormwater wetland surface area, less than 24" deep, and at a higher elevation than the outlet. Please revise in accordance with 15A NCAC 02H .1054(6). RESPONSE: Stormwater wetland geometry was adjusted to meet design criteria. Please refer to sheet LS-3 for revised wetland areas and corresponding percentages. 12. The requisite "deep pools" for the stormwater wetland do not appear to be shown on the plans. Please revise in accordance with 15A NCAC 02H .1054(7). RESPONSE: Please refer to sheet LS-3 for "deep pool" areas and corresponding percentages. 13. Sod is not an acceptable planting for a stormwater wetland. Please delineate the planting zones and select appropriate plants in accordance with 15A NCAC 02H .1054(12)(a), 15A NCAC 02H .1054(13), and 15A NCAC 02H .1054(14). RESPONSE: Sod was replaced for FS (Fox Sedge) along the wetland perimeter. Tall Fescue (Sod) is proposed in the grassed swale and other upland areas near the wetland. Please refer to sheet LS-3 for planting zones. 14. Please provide a trash rack or other device to trap debris on M.E.S.-2 in accordance with 15A NCAC 02H .1054(17). RESPONSE: Trash rack is now proposed on MES-2. See detail on sheet C4.0. 15. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other documents. Pdfs must be uploaded using the form at: https://edocs.deg.nc.gov/Forms/SW Project Submittal RESPONSE: Acknowledged. Response to Review Comments While it is not required, it is recommended that the off -site drainage from upstream properties be diverted around the BMPs since the applicant cannot maintain control over the development of property that they do not own. RESPONSE: Due to the hilly topo and dense wood on the east (back) of the property, it was not feasible to divert all the off -site runoff from that area and prevent it from getting into the BMPs. Please note that this request for additional information is in response to a preliminary review. The requested information should be received by this Office prior to July 10, 2019, or the application will be returned as incomplete. The return of a project will necessitate resubmittal of all required items, including the application fee. Please reference the State assigned project number SW3190501 on all correspondence. If you need additional time to submit the information, please submit your request for a time extension to the Division at the contact below. The request must indicate the date by which you expect to submit the required information. The Division is allowed 90 days from the receipt of a completed application to issue the permit. The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A.
