This letter is in reference to your permit major modification request received June 21, 2018.
In a letter dated August 16, 2018, Tessa Monday of the Water Quality Permitting Section's Non -
Discharge Branch asked you to provide additional information (copy of letter attached) no later than the
close of business on September 17, 2018. As of today, the complete information requested has not been
received, therefore, the Division of Water Resources must return your application as incomplete in
accordance with North Carolina General Statute § 143-215.1.
The information requested but not received are the following:
A. Permit Schedules / Facility Maintenance (per inspection on 9/32IZ
1. Submit a certification (attached to previous permit) from a licensed North Carolina
Professional Engineer certifying that the permitted facility has been installed in accordance
with the provided engineer drawings and specifications.
2. Provide GW-1 forms for all wells that have been constructed. Ensure the coordinates are
correct.
3. The permit requires an ORC and back-up ORC.

B. Operation and Maintenance Plan (15A NCAC 02T 0707)•
1. Provide an addendum to the O&M Plan addressing maintenance of the vegetated fields
(mowing, reseeding, etc.).
C. Site Map (15A NCAC 02T .0704(d))•
1. The location and identity of each monitoring well.
2. The location and ownership identification of property boundaries within 500 feet of the
disposal areas (including road/rail right-of-way).
3. The location of all wells, streams (ephemeral, intermittent, and perennial), wetlands,
springs, lakes, ponds, and other surface drainage features within 500 feet of all facilities.
4. The identification of all approved setback waivers, if applicable.
Please be advised that construction and/or operation of the subject facilities without a valid permit
is a violation of North Carolina General Statute § 143-215.1 and may subject the owner/operator to
appropriate enforcement actions in accordance with North Carolina General Statute § 143-215.6A-6C. Civil
penalties of up to $25,000 per day per violation may be assessed for failure to secure a permit required by
North Carolina General Statute §143-215.1.
When the requested additional information has been obtained, a new application package and
appropriate permit processing fee may be submitted.

Division of Water Resources' Central and Regional staff has reviewed the application package
received June 21, 2018. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on September 17, 2018.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items,
or failure to provide the additional information on or before the above requested date may result in your
application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

1. Provide an as -built for the treatment facility (does not match what was permitted and the facility
description) and field layout. This should include engineering plans, specifications (if different
from the original), and an O&M Manual (if different from the original).
2. Provide GW-1 forms for all wells that have been constructed. Ensure the coordinates are correct.
3. The permit requires an ORC and back-up ORC. An ORC designation form has been provided.

1. Provide an addendum to the O&M Plan addressing maintenance of the vegetated fields (mowing,
reseeding, etc.).
C. Site Map (15A NCAC 02T .0704(d)):
1. Provide a site map showing:
a. The location and identity of each monitoring well.
b. The wetted perimeter of all irrigation fields.
c. The location and ownership identification of property boundaries within 500 feet of the
disposal areas (including road/rail right-of-way).
d. The delineation of compliance and review boundaries.
e. The location of all wells, streams (ephemeral, intermittent, and perennial), wetlands, springs,
lakes, ponds, and other surface drainage features within 500 feet of all facilities.
f. The identification of all approved setback waivers, if applicable.
