I inspected the Old Dominion Freight facility on June 21, 2019. 1 met with Randy Adams on site. During the visit, the
following items were noted:
1) While "New or Proposed Facility' was checked for question 5, the facility has been in operation since 2014.
2) While the NOI only lists one outfall, it potentially has 2 or more. They will confirm this after they produce the
Site Plan.
3) They listed "FBR" as the receiving water body classification on the NOI. I assume this is an acronym for French
Broad River. I Informed Mr. Adams that the question was asking for the actual classification of the French Broad
River.
