DANIEL SMITH Interim Director Carlos Barrios, PE 605 Delaney Ave. Suite C Orlando, FL 32801 NORTH CAROLINA Environmental Quality June 10, 2019 Subject: Request for Additional Information Stormwater Permit No. SW3190501 United Parcel Service Iredell County Dear Mr. Barrios: The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater Management Permit Application for the subject project on May 6, 2019. A review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: Please include the stream name and classification for where the roadside ditch ultimately outfalls to on the application (Section IV, 10, Receiving Stream Name). 2. Please include only the area that drains to the BMP, not the entire property area, in the drainage area section of the application (Section IV, 10, Total Drainage Area). Please also recalculate the values for on -site and off -site drainage areas to reflect this change. 3. Please complete a Supplement EZ form for each BMP. The Supplement EZ form can be found here: https://deq.nc.gov/about/divisions/energy-mineral-land-resources/energy- mineral-land-permit-guidance/stormwater-bmp-manual (Section V). 4. Please provide signed, sealed, and dated design calculations (Section VI, 7). Calculations should include, but are not limited to: a. Calculations showing that the rip -rap at the inlet of the stormwater wetland and lack of rip -rap downstream of M.E.S.-3 are sufficient to prevent erosion during the peak flow from the 10-year storm (As per 15A NCAC 02H .1050(4)). b. Calculations showing that the 10-year storm will be passed at a non -erosive velocity (As per 15A NCAC 02H .1061(5). 5. Please delineate the wetlands on the plans or provide a note stating that no wetlands exist. Provide documentation of the qualifications and identity of the individual who made this determination (Section VI, 8, m). 6. Please show the drainage areas for the BMPs on the plans (Section VI, 8, o). 7. Please provide a copy of the current property deed (Section VI, 10). 8. Please provide official documents with name, title, and position for those listed in Section III of the application (Section VI, 11). 9. The spot elevations of the grass swale, 823.70 feet on the downstream side and 824.00 feet (Shown incorrectly in the stormwater management report, page 4, and inferred from cross-section A -A on sheet C2.0) imply that the grass swale has a slope of 0.003 ft/ft. This slope is shown as 0.03% (Equal to 0.0003 ft/ft) on the plans. Please revise for consistency. 10. The stormwater wetland grading shown in the plans does not match the grading shown in the stormwater management report. Please revise for consistency. 11. The forebay area appears to be less than 10% of the stormwater wetland surface area, less than 24" deep, and at a higher elevation than the outlet. Please revise in accordance with 15A NCAC 02H. 1054(6). 12. The requisite "deep pools" for the stormwater wetland do not appear to be shown on the plans. Please revise in accordance with 15A NCAC 02H .1054(7). 13. Sod is not an acceptable planting for a stormwater wetland. Please delineate the planting zones and select appropriate plants in accordance with 15A NCAC 02H .1054(12)(a), 15A NCAC 02H .1054(13), and 15A NCAC 02H .1054(14). 14. Please provide a trash rack or other device to trap debris on M.E.S.-2 in accordance with 15A NCAC 02H. 1054(17). 15. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other documents. Pdfs must be uploaded using the form at: https://edocs.deq.nc.gov/Forms/SW—Project—Submittal While it is not required, it is recommended that the off -site drainage from upstream properties be diverted around the BMPs since the applicant cannot maintain control over the development of property that they do not own. Please note that this request for additional information is in response to a preliminary review. The requested information should be received by this Office prior to July 10, 2019, or the application will be returned as incomplete. The return of a project will necessitate resubmittal of all required items, including the application fee. Please reference the State assigned project number SW3190501 on all correspondence. If you need additional time to submit the information, please submit your request for a time extension to the Division at the contact below. The request must indicate the date by which you expect to submit the required information. The Division is allowed 90 days from the receipt of a completed application to issue the permit. The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A.
