Project Information Attach a vicinity map clearly showing the location of the property with respect to local landmarks such as towns, rivers, and roads. Also provide a detailed site plan showing property boundaries and development plans in relation to surrounding properties. Both the vicinity map and site plan must include a scale and north arrow. The specific footprints of all buildings, impervious surfaces, or other facilities must be included. If possible, the maps and plans should include the appropriate USGS Topographic Quad Map and NRCS Soil Survey with the property boundaries outlined. Plan drawings, or other maps may be included at the applicant's discretion, so long as the property is clearly defined. For administrative and distribution purposes, the USACE requires information to be submitted on sheets no larger than 11 by 17-inch format; however, DWQ may accept paperwork of any size. DWQ prefers full-size construction drawings rather than a sequential sheet version of the full-size plans. Explain the purpose of the proposed work: Holly Ridge needs expansion of existing sewer treatment plant to resolve a Special Order of Consent from NCDENR to et adequate capacity. Force main to carry treated effluent from the Town of Holly Ridge treatment plant to the proposed spray irrigation fields Spray irrigation alternative will replace the direct Discharge of effluent to tributary of Kings Creek IV. Describe previously approved wetland, stream and buffer impacts, along with associated mitigation (where applicable). If this is a NCDOT project, list and describe permits issued for prior segments of the same T.I.P. project, along with construction schedules. Impact Justification (Avoidance and Minimization) Specifically describe measures taken to avoid the proposed impacts. It may be useful to provide information related to site constraints such as topography, building ordinances, accessibility, and financial viability of the project. The applicant may attach drawings of alternative, lower-impact site layouts, and explain why these design options were not feasible. Also discuss how impacts were minimized once the desired site plan was developed. If applicable, discuss construction techniques to be followed during construction to reduce impacts. Mitigation DWQ - In accordance with 15A NCAC 2H .0500, mitigation may be required by the NC Division of Water Quality for projects involving greater than or equal to one acre of impacts to freshwater wetlands or greater than or equal to 150 linear feet of total impacts to perennial streams. USACE - In accordance with the Final Notice of Issuance and Modification of Nationwide Permits, published in the Federal Register on March 9, 2000, mitigation will be required when necessary to ensure that adverse effects to the aquatic environment are minimal. Factors including size and type of proposed impact and function and relative value of the impacted aquatic resource will be considered in determining acceptability of appropriate and practicable mitigation as proposed. Examples of mitigation that may be appropriate and practicable include, but are not limited to: reducing the size of the project; establishing and maintaining wetland and/or upland vegetated buffers to protect open waters such as streams; and replacing losses of aquatic resource functions and values by creating, restoring, enhancing, or preserving similar functions and values, preferable in the same watershed. If mitigation is required for this project, a copy of the mitigation plan must be attached in order for USACE or DWQ to consider the application complete for processing. Any application lacking a required mitigation plan or NCWRP concurrence shall be placed on hold as incomplete. An applicant may also choose to review the current guidelines for stream restoration in DWQ's Draft Technical Guide for Stream Work in North Carolina, available at hLtp:Hh2o.enr.state.nc.us/iicwetlaiids/stri-iigide.html. 1. Provide a brief description of the proposed mitigation plan. The description should provide as much information as possible, including, but not limited to: site location (attach directions and/or map, if offsite), affected stream and river basin, type and amount (acreage/linear feet) of mitigation proposed (restoration, enhancement, creation, or preservation), a plan view, preservation mechanism (e.g., deed restrictions, conservation easement, etc.), and a description of the current site conditions and proposed method of construction. Please attach a separate sheet if more space is needed. Mitigation of 0.27 acres of permanent impacts will be accomplished by creating 0.5 acres of New wetlands on site. The wetlands created will be adjacent to and connected to existing 404 wetlands on the site. The proposed area is shown on the mitigation drawing M1 The Proposed area will be excavated to the same elevations as the existing and planted with Herbaceous mix of gasses in addition to trees at density of 320 per acre 2. Mitigation may also be made by payment into the North Carolina Wetlands Restoration Program (NCWRP). Stream Crossing 1 Banks have been stabilized and vegetated. Silt fence and rip rap will be removed by hand. Area B Impacts to Area B did not exceed the 40-foot corridor. The area has been returned to natural grade and will be seeded with a herbaceous wetland mix. Area C The entire area will be graded as necessary and seeded with a herbaceous wetland seed mix. Areas beyond the 40-foot corridor will be planted with wetland trees on 10- foot centers as shown on the planting plan. Areas D - P These areas are located within the pre-existing CP&L right-of-way. The areas have been returned to previous grade and seeded with rye. A second seeding with a herbaceous wetland mix will follow. Stream Crossings 2-4 After stabilization and vegetation of the banks, silt fencing will be removed. Area Q Wetland impacts in Area Q occur outside the 40-foot construction corridor. This area will be graded as necessary, seeded with a herbaceous wetland mix, and planted with wetland trees on 10-foot centers as shown on the planting plan. Areas R-T These areas will be returned to original grade and seeded with a herbaceous wetland mix. Areas beyond the 40-foot corridor will be planted with wetland trees on 10-foot centers as shown on the attached planting plans. Areas U / X This area will be returned to original grade to the extent possible and seeded with a herbaceous wetland seed mix. Areas beyond the 40-foot corridor will be planted with wetland trees on 10-foot centers as shown on the attached planting plans. Stream Crossing 5 (Existing) Stream banks were "pulled back" and vegetated for stabilization, however, the culvert pipe was not buried to the required depth and must be removed and replaced according to the attached detail. Temporary silt fencing, check dams and erosion control matting will be used as shown. Stream Crossing 6 Stream banks were "pulled back" and vegetated for stabilization. Velocity dissipaters were installed on upland areas to prevent washout as shown on the attached plan for Stream Crossing 6. Stream Crossing 7 (Main Entrance) Sediment basins within the wetland will be removed and replaced on upland areas. Riprap at the pipe outlet will remain and has been included in calculations for permanent fill. Wetlands will be returned to previous grade and seeded with a herbaceous wetland mix. Areas beyond the 40-foot corridor will be planted with wetland trees on 10-foot centers as shown on the attached planting plans. Restoration Plan Notes 1. The restored sites of this project shown in the restoration plans will be monitored for a minimum of three years for survival and maturity. The sites should have a density of 320 trees per acre minimum. 2. Plants and seeds used should be native species and produced in the geographical region where they will be used. 3. Trees will be balled and burlapped or container-grown. 4. Slow-release fertilizer, such as Osmocote will be used recommended for plants (3oz for 1St gallon and 1 oz for each additional gallon). 5. Planting of trees should occur within 60 days of approval. 6. Seeds sown will be "pure live seed" (pis). 7. Herbaceous wetland seeding rate should be between 50 pis and 100 pis per square foot due to the small size of most wetland species seed. 8. Seed will be mixed with sand (or other "inert diluent") to ensure even distribution. 9. Seeds should be surface-sown, then pushed into contact with the substrate using an ATV and drag or other appropriate light-weight equipment, including hand raking. 10. Seeds should be planted "in the dry" whenever possible and pre-soaked if they will be sown in water. Mitigation DWQ - In accordance with 15A NCAC 2H .0500, mitigation may be required by the NC Division of Water Quality for projects involving greater than or equal to one acre of impacts to freshwater wetlands or greater than or equal to 150 linear feet of total impacts to perennial streams. USACE - In accordance with the Final Notice of Issuance and Modification of Nationwide Permits, published in the Federal Register on March 9, 2000, mitigation will be required when necessary to ensure that adverse effects to the aquatic environment are minimal. Factors including size and type of proposed impact and function and relative value of the impacted aquatic resource will be considered in determining acceptability of appropriate and practicable mitigation as proposed. Examples of mitigation that may be appropriate and practicable include, but are not limited to: reducing the size of the project; establishing and maintaining wetland and/or upland vegetated buffers to protect open waters such as streams; and replacing losses of aquatic resource functions and values by creating, restoring, enhancing, or preserving similar functions and values, preferable in the same watershed.
