Dear Ms. Maddox:
The Pretreatment, Emergency Response and Collection Systems (PERCS) Unit of the Division of Water
Resources has reviewed the new Industrial User Pretreatment Permit (IUP) submitted by the City of Shelby
for the following Significant Industrial User (SIU). This IUP was received by the Division on May 6, 2019.
Additional information was requested and was received on various dates, with a modification received on
June 20, 2019.

The review indicates that the IUP is adequate and the minimum requirements of 15A NCAC 2H .0905 and
.0916 and 40 CFR 403.8(f)(1)(iii) are met. The allocation table, maintained in the Division's database, has
been updated accordingly. Please submit a copy of the inspection to the PERCS file once it is completed.
Federal and State pretreatment regulations require that the local delegated pretreatment program
effectively control and document the discharge of wastewater from Significant and Categorical Industrial
Users to the POTW. It is the POTW's responsibility to ensure that these objectives are consistently met.
