Dear Mr. Harris:
Ron Boone, of the NC Division of Water Resources (DWR), Winston-Salem Regional Office,
visited your home on June 3rd, 2019, to perform a compliance evaluation inspection of your home's
discharging single family wastewater treatment system. Details of that inspection are included on the
attached EPA Water Compliance Inspection Report.
As discussed during the inspection, please get a copy of your current deed to provide proof
of transfer of ownership to you so the permit can be placed in your name. Please provide the
requested deed copy as soon as possible. We cannot put the permit in your name without the deed.

Gregory C. Harris is the current property owner. A change of ownership from previous owners, Ken
and Laura Griffin, was never accomplished. Met with Mr. Harris. He is aware that he has a septic tank
and a chlorinator but not the location of the outfall or any of the other requirements. I left him with a
packet of information including the general permit, technical bulletin, the type of chlorine to use and
where to buy it, local laboratories, and provided advice on finding the outfall. Also briefed him on
significant changes to permit language in last rewrite (i.e. checking septic tank solid's level annually).
We also filled out a NOC form and he signed it. He said he would get a copy of the deed for me to send
to CO with the NOC form as soon as possible.
