SUBJECT: NOTICE OF DEFICIENCY

Dear Mr. Smith:
The North Carolina Division of Water Resources conducted a Pretreatment Audit inspection of the Hendersonville
WWTP on June 11, 2019. This inspection was conducted to verify that the facility is operating in compliance with
the conditions and limitations specified in NPDES WW Permit No. NC0025534. The purpose of the audit is to
determine the effectiveness of the pretreatment program by an in-depth review of the files, including the industrial
monitoring data, appropriate data management and adherence to the Enforcement Response Plan. Please see the
enclosed Pretreatment Audit Inspection Form for more detailed information about the inspection. The program
was given a marginal rating and the following actions need to be completed by 7/19/2019:

Pretreatment An ownership change IUP modification needs to be submitted for GE Lighting to Vivien Zhong. _________________________________________________________________________________________________________________________________________________________________________
Pretreatment Enforcement Response Plan needs to be sent to all industries. _________________________________________________________________________________________________________________________________________________________________________
Pretreatment GE Lighting needs to receive an NOV for their March 2018 sample violation. _________________________________________________________________________________________________________________________________________________________________________
Pretreatment Update the Long Term Monitoring Plan to reevaluate the ammonia detection le vel and SIU
Sampling frequency and submit it to Vivien Zhong. _________________________________________________________________________________________________________________________________________________________________________
In addition, the actions below should also be taken:
1. Request a copy of the Toxic Organic Management Plan from from GE Lighting.
2. Request a copy of the Slug Spill Control Plan from Kimberly-Clark.

Remedial actions should have already been taken to correct these problems and prevent further occurrences in the
future. The Division of Water Resources may pursue enforcement action for this and any additional violations of
State law. To prevent further action, carefully review these deficiencies and address the causes of non-compliance
to prevent the recurrence of similar situations.
