Project Summary
The applicant plans to build a 5,160 SF worship center at their site in Durham, with provision for a Sunday School classroom,
with an assembly space of 1,652 Square feet, accommodating 236 worshippers (@3 GPD, 708 GPD), as well as a classroom
that will accommodate 21 students plus one teacher (@10 GPD, 220 GPD). The calculated total load of 928 GPD with peak
factor of 2.5 is 2400 GPD. This effluent is intended to discharge into a Durham County maintained sewer system.
Due to the topography of the site and the location of the building close to Page Road, the sewer line has two types of effluent
flow. The first 552 feet is through a 6" PVC pipe with gravity flow passing through one cleanout and two manholes and
discharge to a lift station, and a 2" PVC Schedule 80 force main that is 1,020 feet discharges into a Cleanout 10 feet from
property, from where a 6" PVC pipe with gravity flow passes to a second Cleanout at the Property line, which then flows
through a 6" PVC pipe into Durham County maintained and operated area on Sounding Lane at Manhole No. T-16-115.
The lift station is equipped with two pumps each with 44 GPM capacity, or a total capacity of 88 GPM, pumped through a 2"
PVC Schedule 80 pipe, with one air pressure relief valve. The total dynamic head is computed to be 57.15 feet and other
related information is provided on the drawings for the lift station. A 30'-0 Private Easement is dedicated for this sewer line.
A 24 hour leak test shall be conducted with Durham County supervision before the system is put into service.
The gravity line as well as the force main maintain the required regulations traversing to the final discharge point. The
applicant requests a sewer extension permit for this sewer line, so that they may have the permit to start construction.

III. Certification Statement:
I Stephanie Brixey certify to the best of my knowledge that the addition of
the volume of wastewater to be permitted in this project has been evaluated along the route to the
receiving wastewater treatment facility and that the flow from this project is not anticipated to
cause any capacity related sanitary sewer overflows or overburden any downstream pump station
en route to the receiving treatment plant under normal circumstances, given the implementation of
the planned improvements identified in the planning assessment where applicable. This analysis
has been performed in accordance with local established policies and procedures using the best
available data. This certification applies to those items listed above in Sections I and II plus all
attached planning assessment addendums for which I am the responsible party. Signature of this
form indicates acceptance of this wastewater flow.

IFB 18-029: Stirrup Iron Creek Lift Station is in design or under construction with
Capacity_ Improvements (WQ0039913)
planned completion in August 2019 ; and/or

concerning two pump failures at the station. The current firm capacity is based upon the
160 hp bypass pump being in operation with the 70 hp pump.
_Notice to Proceed issued to Carolina Civil Works on January 7, 2019. Substantial
completion required by September 29, 2019.
Once the capital project is completed, the firm capacity will be 6.5 MGD.
Therefore:
Given reasonably expected conditions and planning information, there is sufficient justification to
allow this flow to be permitted, without a significant likelihood of over -allocating capacity in the
system infrastructure.
I understand that this does not relieve the collection system owner from complying with G.S. 143-
21 5.67(a) which prohibits the introduction of any waste in excess of the capacity of the waste
disposal system.

Instructions for Flow Tracking/Acceptance form (FTSE) and Planning Assessment Addendum (PAA)
Section I
a. WWTP Facility Name: Enter the name of the WWTP that will receive the wastewater flow.
b. WWTP Facility Permit M Enter the NPDES or Non -Discharge number for the WWTP receiving the
wastewater flow.
c. WWTP facility's permitted flow, MGD: From WWTP owner's NPDES or Non -Discharge permit.
d. Estimated obligated flow not yet tributaa to the WWTP MGD: This includes flows allocated to other
construction projects not yet contributing flow to the collection system. Flows allocated through
interlocal agreements or other contracts not yet contributing flow to the collection system are also
included. For POTWs that implement a pretreatment program, include flows allocated to industrial
users who may not be using all of their flow allocation. Please contact your Pretreatment Coordinator
for information on industrial flow tributary to your WWTP.
As of January 15, 2008 the POTW should have reviewed flow allocations made over the last two years
and reconciled their flow records, to the best of their ability, so it is known how much flow has been
obligated and is not yet been made tributary to the WWTP, in accordance with local policies and
procedures employed by the reporting entity.

is 0.5 MGD of flow that is obligated but not yet tributary. The annual average flow for 2007 is 2.7
MGD. There is a proposed flow expansion of 0.015 MGD.
The first Form FTSE submitted after January 15, 2008 may have numbers like this:
c. = 6.0 MGD
d.=0.5 MGD
e. = 2.7 MGD
f. = 0.015 MGD
g. = 3.215 MGD
h. = 53.6 %
The next Form FTSE may be updated like this with a proposed flow expansion of 0.102 MGD:

Jason,
I do not have a firm date yet on the 200 HP pumps. The bids are due on 6/28. The contractor will have 240 days from the
notice to proceed to reach substantial completion. So, we are looking at 8 —10 months out before the two additional
pumps are installed and operational. The pumps have a lead time of about 6 months.
As for the timeline with the upcoming projects, I'm unsure. I will have to check with the developers. I'll make sure to
include a letter outlining the situation for all sewer extension permits tributary to the Stirrup Iron Pump Station.

Yes, the Firm Capacity should be the pump capacity with the largest pump out of service, including the bypass pump.
And whether the available capacity is positive or negative on the FTSEr the "Planning Assessment Addendum" on the
FTSE should be completed since this situation is in in flux. The situation should also be mentioned in the narrative for the
project.
Do you know when the two additional 200 HP pump be installed? Would it be before a newly permitted project would
be built and be put into service?

Jason,
I was curious on how to handle FTSE permitting associated with Stirrup Iron Pump Station going forward as the number
and type of pumps have changed due to the failure of both 200 HP pumps.
In a week or two, we will receive one of the 200 HP pumps back from the repair shop. The other 200 HP pump is not
repairable. After the 200 HP pump is reinstalled, we will have keep an additional submersible bypass pump onsite to
maintain redundancy. Based on drawdowns, the submersible bypass pump pumps at a rate less than the 200 HP pump
(I'm waiting to get a confirmed number from our maintenance staff).
Would the Firm Capacity be the capacity with the largest pump out of service? Is this ok even with the bypass pump?
In the event it shows that we have negative capacity at the station, will it be ok to submit FTSE applications showing the
negative number as we are in the process of adding two additional 200 HP pumps (WQ0039913)? i.e. Will they be
approved, even if conditional?
