The only version we have is a scanned and printed copy, not the original signed copy. The deed restrictions form needs to be revised because the legally responsible party is still Dennis Moser of Liquid Management, LLC. Send all suspicious email as an attachment to report.spam@nc.gov Suzanne, I believe Corey is stating below that he only received a copy of the permit transfer form. Our client resent the original permit transfer form, $505 fee check, and Operations & Maintenance Agreement back to your office on 3/26/19. You then received said package on 3/27/19. Per the attached email you sent on 3/19/19, there was no mention of the deed restrictions form that needed to be revised and sent for permit transfer. Can you provide comment on this? I was able to look over everything that you sent in. Our regulations require that the transfer application that is submitted be the original signed application, not a copy. Could you please send me the original signed copy? Also, we need an updated version of the deed restrictions and protective covenants form signed by the new owner.
