4. Project Description and History
4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The wasting conditions onsite include an wasting 30" in diameter x 68' in length RCP which conveys waters of an Unnamed Tributary (UT) to Panther Branch beneath SR 1779 Eldon
Thornton Road. The waters of subject stream are routed through an epsting maintained lateral roadside ditch for appropmately 260 feet until connection back to the natural system.

The purposed of the project is to restore the conveyance of stream waters beneath SR 1779 Eldon Thornton Road and through the jurisdictional lateral roadside ditch to its original
capacity.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
The project includes the proposal to replace the deteriorated 30" in diameter x 68' in length RCP With a new 66" in diameter x 68' in length CMP and excavate the existing lateral
roadside ditch which has accreted sediment, vegetation, and debris which is backing up water exiting the outlet of the epsting crossline pipe. Sheetpile will be installed accross the
stream channel and ditch and bypass pumping Will be utilized for site dewatering. Water from within the construction zone will be pumped into a silt bad for treatment prior to discharge
back into the system. See attached profile view for existing and proposed ditch grade.

Proposed dewatering impacts are necessary for the replacement of the existing crossline. Excavation of the lateral roadside ditch will promote better
conveyance and prevent future flooding of the existing road and erosion and scour around the existing structure.

Project will dewater the minimum necessary for installation of the replacement crossline. Proposed excavation of the stream will be limited to a portion
that is within the existing maintained roadside ditch and will be beneficial by restoring normal flows, connectivity, and aquatic passage.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
Strict adherence to applicable permit conditions and implementation of sediment and erosion control BMPs in accordance with NCDOT BMP manual.
