REGAN Secretary LINDA CULPEPPER Director Wally Hansen, Public Services Director City of Jacksonville PO Box 128 Jacksonville, NC 28541-0128 SUBJECT: NOTICE OF DEFICIENCY Dear Permittee: NORFH CAZO_INA Envirc, nmerrud 2U01ily June 11, 2019 Tracking Number: NOD-2019-DV-0009 Sanitary Sewer Overflows — May 2019 Collection System Permit No. WQCS00268 Jacksonville Collection System Onslow County The self -reported Sanitary Sewer Overflow (SSO) 5-Day Report submitted by the City of Jacksonville indicates a violation of permit conditions stipulated in the subject permit and North Carolina G.S. 143-215.1. Violations include failing to effectively manage, maintain, and operate the subject collection system so that there is no SSO to the land or surface waters and if applicable making an outlet to waters of the State for purposes of G.S. 143-215.1(a)(1), for which a permit is required. Specific incident(s) cited in the subject report include the following: Total Incident Start Vol Number Date Location Cause (Gals) DWR Action 201900965 5/23/2019 MH at Brynn Marr PS Station Failure 2,800 Notice of Deficiency The filed report indicated that your SCADA system detected a communications failure with the Brynn Marr station at about 1:30 am. However, based on the report it appears that the City staff did not investigate the situation until 7:40 am. If communication is not restored within the timeframes in your pump station spill & failure plan for this specific station, then staff should be dispatched to hopefully avoid or minimize a spill. The City was very fortunate this incident was apparently only minimal. Remedial actions, if not already implemented, should be taken to correct the above noncompliance. Failure to do so will be a mitigating factor in a subsequent incident. 127 Cardinal Drive Extension, Wilmington, North Carolina 28405 Phone: 910-796-7215\ Internet: www.deg.nc.gov DocuSign Envelope ID: 1D542943-61EE-4F94-84E9-A123525909F7 If you have any questions, please do not hesitate to contact Dean Hunkele with the Water Quality Section in the Wilmington Regional Office at 910-796-7215 or via email at dean.hunkele@ncdenr.gov.
