Mr. Mahan further alleges that all this material is stopped -up at the dam that he runs. Dam is privately owned by W 1 Lee. Action Taken: DWQ visited sight on the morning of 8/20108 since that was our first available date. The poultry house was visited and up/down stream samples obtained for analysis. Incident Questions: Did the Material reach the Surface Water? No ,Surface Water Name? Did the Spill result in a Fish Kill? Unknown If the Spill was from a storage tank indicate type. Containment? Unknown Cleanup Complete? Unknown Water Supply Wells within 1500ft : Unknown Directions: Poultry house is owned by Ronnie and Karon Lashaw of 3736-C Claremont Trail, Graham, NC 27253. Poultry house is located at this address. Comments: No evidence of poultry litter or feathers outside the 1025'x45'pouftry house. Vegetation was chest high around the poultry house and large overhead doors had woody vegetation growing in front of them. Per owner of property (Ronnie Lashaw 336.269.0024) most all birds and litter were removed by Justin Donley (leasee) by February of 2008. No waste was applied onto land at this address. Only 98 birds remain. No odors or noise observed around poultry house. No DO or pH violations in Haw River upstream or downstream of the poultry house. Upstream fecal was 30 and downstream fecal result was 23. No feathers or solid waste evident at the upstream or downstream sampling sites. Poultry house was built in 1995 and is located 1795 feel from the Haw River at Its closest point, through extremely dense vegetation (not drivable).
