Dear Permittee:
The North Carolina Division of Water Resources conducted an inspection of the Flesher's Fairview Rest
Home on 6/03/2019. This inspection was conducted to verify that the facility is operating in compliance
with the conditions and limitations specified in NPDES WW Permit No. NC0034924. The findings and
comments noted during this inspection are provided in the enclosed copy of the inspection report
entitled "Compliance Inspection Report".
Please respond in writing to this office within 30 days of your receipt of this letter regarding your plans or
measures to be taken to address the following issues:
Disposal of debris (specifically noted were wipes) is not occurring properly throughout this establishment. This
could cause expensive infrastructure damage, health concerns for residents/staff and environmental damage.
All wipes used at Flesher's Rest Home are to be place in a trash receptacle by staff and residents after use, not
flushed down the toilet.
Flushing of these wipes is problematic for several reason; one problem, especially in summer months, is vector
control. Another is wipes damage equipment and clog sewer lines because they do not breakdown (dissolve in
water) like toilet paper. Just because they are advertised as "flushable" they should never be flushed. The
only thing that is allowable for flushing is toilet paper.
The ORC must have garbage bags/garbage can down at the wastewater treatment plant for proper debris
disposal in the facility's dumpster.

Linda Wiggs (Inspector) met onsite with Mark Jones (ORC) and Daniel Jones. The ORC has
conscientiously been addressing items from the previous inspection report.
The solids management actions taken by the ORC appear to be affective. On the day of the inspection
the settleability test was 40%, blankets were at 4' and there was no build up in the contact chamber.
The ORC is wasting as settleability test indicates needed; wasted March 6, April 6, May 6. The
Digestor was pumped March 13. It is due for pumping again ASAP.
Owner Immediate Action Items:

The influent basin has excessive wipes in a scum layer across 3/4 of the basin. This must be removed
and not allowed to build up. The appropriate way for this to be minimized is to not flush wipes down the
toilet. All wipes are to be place in a trash receptacle by staff and residents.
Flushing of these wipes is problematic for several reason; one problem, especially in summer months,

garbage.
Air:
The ORC stated the D.O. was 2.6 mg/I on May 21.
Air On 15/off 45. Air appeared to billow near the air line, diffuser caps may need replacement. ORC
needs to check the D.O. more frequently and adjust air/time as needed. Suggested at least weekly in
each basin, with air on and off.
Digester/Influent Basin/Septic Hauler:
Debris is not to be disposed of in the digester. The digester needs to be pumped again ASAP. The
scum/wipe matt in the influent basin needs to be removed ASAP and moving forward the debris/wipes
needs to be managed appropriately.

pumped again ASAP. Last haul was March 13. Increased pumping during summer months
