This means that your permit was not renewed. A fees owed letter was sent out last September (attached), but if we did
not receive payment ahead of the renewal, the coverage was not renewed. I don't have access to the payment date
records for this permit, so I cannot tell when the fees were paid.
Deborah, can you tell whether any annual fee payments were late for this permit in the past?
I see that fees are now all current (see below), and so it can be renewed. (If the permittee does not notify us when all
fees are paid up, we do not know to renew the COC, and it stays expired.) I am copying Suzanne McCoy so she can take
care of the renewal and send you a new COC. We won't be able to update the on-line system (because those data were
taken at the time of renewal and will not be updated with info on permits that were paid later).

I recently took overt he HSE duties for Siegwerk— NCG090032 and have discovered that we
are delinquent on our Stormwater permit. I have spoken with Mr. Reed at the regional office
and he directed me to complete the COC renewal request. At the bottom of the form it
indicates that the permit has expired, which I am aware of, but it will not allow me to enter
anything in the Issuance date field.
I assume this is operator error so any guidance you could provide would be great.
