Dear Mr. Kennedy:
On May 29, 2019, Paul DiMatteo of the Division of Water Resources (Division), Winston-Salem
Regional Office met with Jerry Kennedy, Operator in Responsible Charge (ORC), Charlie Brushwood,
backup-ORC, and Logan Storey to conduct a Compliance Evaluation Inspection of the above referenced
Wastewater Collection System.
This inspection consists of (1) a review of the permit and accompanying documentation
to determine compliance with permit conditions, and (2) an on -site inspection of several
collection system components. The attached inspection form notes the areas that were
evaluated for the inspection, with notable findings outlined as follows.
Site Inspections
I. Pump Stations — The Northwest and Fryes Creek pump stations were inspected and no issues
were noted.
2. High Priority Lines — The Millers Creek Outfall was inspected and no issues were noted.
3. Manholes —A manhole nearthe Millers Creek Outfall was inspected and no issues were noted.

Inspection Summary:
On May 29, 2019, 1 met with Jerry Kennedy, Logan Storey and Charlie Brushwood to complete a compliance evaluation
inspection of this collection system. Jerry, ORC, said he was retiring at the end of June and wanted to do an inspection with
his replacement, Logan, before he retired.
No items of noncompliance were noted during the inspection. The system hasn't experienced a reportable overflow since
2010.
