On April 16, 2009 the Division of Water Quality (DWQ) received correspondence dated Apl'il 9,
2009 forthe above referenced project. Please provide the followIng so that we may continue to review
your project

1. Please resubmit five (5) sets 011 the llew PeN farm that Went into affect JanuElrjl 12,2009 610ng with
ltppropriate maps and narrative for the modification.
2. A request for a modification of an issued 401 approval permit requires submission of the nppropriare
fee based on impacts.
Please contact the DWQ within three weeks of the dale of this letter to verifY that you have received
this letter and that you remain interested incontil'luing to pUrSue pennitting ofyotlr project and will be
providiJlg the DWQ the requested illf"rmation at!l Jater date. Please contact tItis office in writing. If we
do not hear from you within three weeks, We will aSSUme that you no longor want to pursue this project
and we will consider t.he project as returned.
