Enclosed is a copy of the lab report from your well taken on March 14, 2011 by Derek Denard and Shuying Wang with
Aquifer Protection Section of the North Carolina Division of Water Quality Winston-Salem Regional Office. In addition,
please see Well Sampling Summary below (page 2) with results compared to groundwater standards. With the exception
of Fecal Coliform and Iron all results met groundwater standards found in the North Carolina Administrative Code 15A
NCAC 02L .0202.
The Iron result was 610 µg/L (the groundwater standard is 300 µg/L). The higher Iron concentration is likely from natural
sources such as the geology of the area or from your plumbing since the sample was taken from a spigot.
The Fecal Coliform result for the well water was 640 colonies/100 mL (the groundwater standard is (colony/100 mL) and
is considered unsafe for human consumption. If you have any concerns about health effects, please contact the N.C.
Occupational and Environmental Epidemiology Branch at (919) 733-3410 or your local health department.
Please note that these results do not indicate a possible source for the Fecal Coliform such as the land application of
animal waste or wastewater residuals. Sources of the Fecal Coliform could be from numerous natural, domestic, and
agricultural sources. These other sources of Fecal Coliform could include pets, livestock, wildlife, and septic systems that
influence groundwater quality nearer the surface.
The presence of Fecal Coliform in your well may be attributed to your well construction. Since your well is a bored type
well, it is most likely not grouted (scaled) from water on the surface. As a result, bored type wells receive groundwater
from sources closer to the surface instead of the deeper aquifer. Well construction standards for Water Supply Wells are
found in North Carolina Administrative Code 15A NCAC 02C .0108. Please see the enclosed brochure "Water Supply
Well Construction" for information on drilling and construction standards for a safe and dependable water supply.

introduction
Groundwater is a source of fresh, unpolluted
water. More than half of the people who live in
North Carolina rely on groundwater wells as their
primary source of household water. Well
pollution that does occur in water supply wells is
typically due to improper well location or well
construction.
The Division of Water Quality's mission is to
protect and enhance North Carolina's surface
water and groundwater resources for the citizens
of North Carolina and future generations. In
conjunction with this mission, the North Carolina
Well Construction Standards (15A NCAC 02C
.0100) were developed to require the location,
construction, repair, and abandonment of wells,
and the Installation of pumps and pumping
equipment to conform to such reasonable
standards and requirements as may be
necessary to protect the public welfare, safety,
health, and ground water resources.
Permits
All wells must conform to minimum specifications
for construction and teiting. Before you can
construct a well, you must first obtain a permit.
Permits for domestic water supply wells are
issued through your' local county health

Certified Well Contractors
If you hire a well contractor to construct, repair or
abandon your domestic water supply well or any
other well, North Carolina iequires that the driller
must be certified by the Stite's Well Contractors
Certification Commission. You can check to see
if your driller is certified by visiting the North
Carolina Well Contractor's Certification
Commission website at:

Water Suooly Well Coristruction
The following are general construction standards
for Water Supply Wells. Construction standards
for wells other than water supply can be found in
15A NCAC 02C .0108.
Location: Water supply wells have set back
requirements to minimize the potential for
contamination to enter the well of seep around
the well and into the groundwater. Some
potential sources of contamination requiring
setbacks are:
• Septic tanks, drainfields, drainfield ir
areas, cesspools, and privies
• Animal barns and feedlots
• Building perimeters, Including attached
structures
• Chemical or petroleum storage tanks
• Landfills
For a complete list of set back requirements,
please refer to 15A NCAC 02C .0107(a).
Casing: A casing Is the pipe or tubing that is
installed in the borehole of the well that helps
support the sides of hole from caving and can
also prevent contamination from entering the
well. In general, a well must be cased with
approved casing to a depth of at least 20 feet.
However, some localities require greater casing
depths and some localities will allow casing to
depth of less than 20 feet. Check with your
Department of Environment and Natural
Resources (DENR) Aquifer Protection Section
(APS) regional office to determine the re ed
casing depth for your area. This info is
also found in 15A NCAC 02C .0116 and�l7.
If the well is constructed to obtain water from a
consolidated rock formation, the casing must be
seated at least five feet Into the consolidated
rock. Also, a drive shoe must be attached to the
bottom of the casing it the casing is driven into
rock. The drive shoe protects the casing from
damage during instatletlon. If the well is

Water Well Disinfection or Chlorination
For many of us, a water supply wall
represents the sole source of water for our
home. Disinfecting your water supply well
and piping system is an effective way to
ensure that your water is sanitary and safe
to drink. Disinfection of a water supply well
is necessary if lest results indicate bacterial
contamination. Chlorination of your well,
piping system, and water heater is also
necessary if your well is contaminated by
flood water. Disinfection may be used to
control iron and sulfur bacteria to a limited
degree. You may also wish to chlorinate
your well as part of an annual well
maintenance practice. In addition, all water
wells are required to be disinfected upon
completion of construction, maintenance,
repairs or pump installation and testing.

The standard method of disinfection
produces a 100 parts per million (or 100
milligrams per liter) chlorine concentration in
your water. Another type of chlorination
termed "shock chlorination" uses the same
methods to introduce chlorine but achieves
at least a 200 parts per million residual
chlorine or more. Shock chlorination is
typically recommended when test results
indicate the presence of bacteria.
REMEMBERI The only people allowed to
break a well seal are the well owner,
plumbers installing or repairing well pumps,
and certified well contractors.

Well
To safely chlorinate your well, you should
use safety goggles, gloves and appropriate
clothing. Follow chlorine product
manufacturer's instructions. Concentrated
chlorine can produce holes in clothing and
skin burns. You can use household bleach
or a solution made from high test calcium
hypachlorite containing 65% - 75% available
chlorine. High-test calcium hypochlorite,
including trade names HTH and Chlor-Tabs,
is available from home improvement stores,
swimming pool product suppliers, and driller
supply stores. Do not use stabilized
chlorine tablets or any chlorine product that
contains fungicides, aigaecides or other
disinfectants; read the product labe
carefully. You may wish to ask the well
contractor that installed your wethe!

Inspection Summary:
Today's inspection was the result of a complaint received from the Davidson County health Dept. regarding a complaint they received
from a Mr. Talbert. Per the County Health Dept., on March 8, 2011 Mr. Talbert brought what was purported to be a sample of his well
water into their office. The sample was reportedly black in color and Mr. Talbert alleged that his well hadbeencontaminated by the
recent application of municipal sludge onto the field next to his property.
Through follow-up conversations between APS-WSRO staff and Mr. Talbert, our office was told by Mr. Talbert that the waste was,
reportedly, animal waste from a nearby cattle farm. Upon further investigation, staff discovered that the application field is owned by
David Leon Phillips, Jr. and received an application of dairy waste from Red Acres Farm, LLC. on March 5, 2010. The application field
in question is Tract-3070 Field-1 and is listed in the farm's Waste Utilization Plan (WUP).
10. Per the operator's records, approximately 164,000 gal. of slurry waste was applied onto 23.21 acres of land to be planted in com
within 30 days. Since the agronomic rate is 197 lbs. N/acre and the waste was applied at only 70lbs. N/acre (based on a waste
analysis of 9.9lbs. N/1000 gal.), there was no overapplication of nitrogen. The field is currently covered in soybean stubble.
Additionally, no run-off of waste from the field was observed. There is a wooded buffer between the field and Mr. Talbert's property
and the waste was applied at least 20 feet away from the property line. The application field was not frozen or wet at the time of
application, however, it did receive 1.3 inches of rain in the 24 hours post -application. All of the operators waste applicationrecords
appeared to be complete and in order."-
DWQ did document, however, that the _animal -waste was —applied approximately 90 feet from Mr. Talbert's well. The farm's WUP states
that waste shall not be applietl any closer than 100 feet to wells. It should be noted that Mr. Talbert's concrete well cover was
observed to be askew and nor properly placed over the well opening. Although no animal waste was detected on Mr. Talbert's
property, the operator of the cattle farm was cautioned about application set -backs in the future.

The installation of the Red Acre Dairy Waste Storage Pond Retrofit in Davidson
County has been completed. This installation meets NRCS design and
specification requirements and is hereby approved for cost share payment.
The work includes installing a second -stage waste storage pond with skimmer pipe
and emergency spillway. In reviewing the design package, I found some
adjustments to be made. These new calculations resulted in altering the crest
elevation of the emergency spillway and the elevation of the "begin pump" max
liquid level marker. Both of these changes were carried out. My revised
calculations are to be filed in the case file. Be sure to complete the As -Built
drawings with all changes indicated in red and file them in the case file as well.

INSTRUCTIONS TO NC PROFESSIONALS:
The classification of the downslope surface waters (the surface waters that any overflow from the facility would flow toward) in
which this animal waste management system will be operated must be determined by the appropriate DWQ regional office.

This letter is being sent to clarify the recordkeeping requirement for Plant Available Nitrogen (PAN)
application on fields that are part of your Certified Animal Waste Management Plan.
In order to show that the agronomic loading rates for the crops being grown are not being exceeded, you
must keep records of all sources of nitrogen that are being added to these sites. This would include nitrogen
from all types of animal waste as well as municipal and industrial sludges/residuals, and commercial fertilizers.
Beginning January 1, 2000, all nitrogen sources applied to land receiving animal waste are required to
be kept on the appropriate recordkeeping forms (i.e. IRR1, IRR2, DRY1, DRY2, DRY3, SLUR], SLUR2,
SLD1, and SLD2) and maintained in the facility records for review. The Division of Water Quality (DWQ)
compliance inspectors and Division of Soil and Water operation reviewers will review all recordkeeping during
routine inspections. Facilities not documenting all sources of nitrogen application will be subject to an
appropriate enforcement action. 4

Senate Bill 1217, An Act -to Implement Recommendations of the Blue Ribbon Study Commission
on Agricultural Waste, was ratified by the North Carolina General Assembly on June 21, 1996.
This bill required that a properly certified operator be designated as the Operator in Charge for each
animal waste management system that serves 250 or more swine, 100 or more confined cattle, 75 or
more horses, 1,000 or more sheep, or 30,000 or more confined poultry with a liquid animal waste
management system.
The deadline for designating an Operator in Charge for animal waste management systems
involving cattle, horses, sheep, or poultry was January 1, 1997. Because a training and
certification program was not yet available for these systems, you were allowed to apply for and
were issued a temporary animal waste management certificate from the Water Pollution Control
System Operators Certification Commission (WPCSOCC). Because you applied for and were
issued a temporary certificate, you were allowed to be designated as the Operator in Charge of an
animal waste management system.
Your temporary certificate expires December 31, 1997, and is not renewable. Our records indicate
that you have not obtained a permanent animal waste management system operator certification. If
you intend to remain the Operator in -Charge of the facility for which you were designated, you must
obtain a permanent animal waste management system operator certification of the appropriate type
before your temporary certification expires on December 31, 1997. To obtain a permanent
certification, you must be 18 years of age, complete ten hours of approved training, and pass an
examination. If you do not intend to remain the Operator in Charge, a properly certified animal
waste management system operator must be designated as Operator in Charge of the facility prior to
the expiration of your temporary certification.
This year's final training program for both Type A and Type B animal waste management systems
is scheduled for December 10 and 11, 1997, at the Hunt Horse Complex in Raleigh. If you would
like information about this training program, please contact your local cooperative extension agent
or call Dee Ann Cooper at 919/515-6968. For those enrolled in this training and registering with
Ms. Cooper prior to December 1, an exam will be offered at the Hunt Horse Complex on December
11, beginning at 1:00 pm.

In addition to the training and testing at the Hunt Horse Complex, examinations for permanent
certification will be offered on December 11, 1997, in Williamston, Kenansville, Raleigh
(Wake Tech), Wentworth, Salisbury and Morganton. This examination date will be the last
opportunity to obtain permanent certification before your temporary certification expires. If you
need additional information or have questions concerning the examinations for certification, please
