On May 2, 2019, Vanessa Manuel from the Raleigh Regional Office conducted an inspection of
the town's wastewater collection system to evaluate compliance with the operation and
maintenance of the subject wastewater collection system permit. The cooperation and assistance
of Thomas Ellis and Anthony Branch, Backup Operator in Responsible Charge, during the
inspection were greatly appreciated.
Our records indicate the wastewater collection system is classified as Grade I and consists of
approximately 10 miles of gravity sewer, approximately 2 miles of force main, 6 duplex pump
stations, and 5 high priority sewer Iines.
The subject permit is effective from June 1, 2018 until May 31, 2026, and is subject to specific
conditions and limitations. The items below show what conditions were noted during the
inspection:
1. Reported Sanitary Sewer Overflows. During the period May I, 2017 through April 11,
2019, there were 13 SSOs from the town's wastewater collection system with a combined
estimated volume of 68,625 gallons, of which 64,075 gallons reached surface water. Ten
of the 13 SSOs, resulted in this office issuing the Town Notices of Violation. No fish
kills were reported because of the SSO events.
2. Fats, Oils and Grease Program. The Town of Spring Hope, hereinafter referred to as
the Town has developed and implements an educational fats, oils and grease program that
includes at Ieast bi-annual distribution of educational material targeted at both residential
and non-residential users. FOG educational material was mailed to customers on March
22 and August 30, 2018. The Town has also developed and implements an enforceable

FOG program for non-residential users. The operators were not aware of any
enforcement actions taken against non-residential users.
3. Capital Improvement Plan. The Town has adopted and implements a Capital
Improvement Plan to designate funding for reinvestment into the wastewater collection
system infrastructure. The CIP covers a 20-year period from 2004, includes a description
of the project area, description of the existing facilities, known deficiencies and
forecasted future needs. The approximate capital improvement budget for the collection
system for fiscal year 2018 was $471,750.
4. Contingency Plan for Pump Failure. The Town maintains a contingency plan for pump
failure at each lift station. A copy of the Plan was submitted with the Town's permit
renewal application package received December 15, 2017.
5. Pump Stations Inspected. The following 5 of the Town's 6 duplex pump stations were
inspected: US 64 Alternate Lift Station, Ryan Court Lift Station, 581 North Lift Station,
Montgomery Lift Station, and Country Inn Lift Station. The inspectors' observations
noted in the next 5 items refer to the 5 pump stations inspected.
6. Pump Station Signage. Each pump station has signage clearly and conspicuously posted
that identifies the pump station, provides an emergency 24-hour contact telephone
number(s), and provides instructions for notification if the visual alarm illuminates and/or
the audible alarm sounds, or if an emergency is apparent.
7. Pump Station Restricted Access. The pump station sites, equipment and components
were observed to have restricted access.
8. Pump Stations Automatic Polling Feature. The pump stations are equipped with
simple telemetry that will notify the Town in the event of power loss or if the high water
alarm is activated. During the inspection, the telemetry system acknowledged the alarms
when the high water float was activated.
9. Pump Station High Water Alarms. The pump stations are equipped with audible and
visual high water alarms. During the inspection, these alarms functioned properly at the
US 64 Alternate and Country Inn Lift Stations. At the Montgomery Street Lift Station,
the visual alarm did not function and at the Ryan Court and 581 North Lift Stations, the
audible alarms did not function. In addition, the location of the visual alarm at the Ryan
Court Lift Station is low and not visible from the road near the school. Section I.10 of
the subject permit requires the alarms to be easily seen and visible. Within 45-days of
receiving this letter, please update this office regarding the status of the visrtal and
audible alarms, the cause of the alarm failures, and the actions the To►wi will
implement to ensure all alarms function in the event of high ►vter in the wet ►vells.
10. Pump Station Inspection Program. The Town has developed a routine inspection of
each pump station. The inspector observed the following notations within the pump
station logbooks: date of operator's visit; operator's initials; and pump hours. Some of
the logbooks observed reflected less than weekly operator visits. The logbook at the
Ryan Court Lift Station indicated operator visits on February 5, 12 and 19, 2019, April 3
and 8, 2019. No visits were indicated for the weeks of February 20, April 15th, 22"d or

February 5 and 19, March 5, 12, and 19, April 8, 19, and 29, 2019. No visits were
indicated for the weeks of February 12`h or 26`h, March 251h, April I" or 22nd. Within 45-
days of receiving this letter, please update this office regarding life staters of the Town's
pump station inspection prografn. Please ensure the mininuutt maintenance activities
required by the subject permit are incorporated into the Town's inspection progrant of
the lift stations and duly noted in the logbooks, and ensure the lift stations are
inspected at least once a week.
11. Designated Operators. The Town's wastewater collection system is classified as a
Grade I collection system (CS-1). Currently, the Town does not have a designated
certified Operator in Responsible Charge (ORC). The back-up ORCs on record are
Anthony Branch, Dean Allan Holmes, Steven Andrew Sherrod, and Jeffrey Bruce
Whitley. The certification statuses for each designated back-up ORC are reflected as
active in the Division's data system. North Carolina Administrative Code Title 15A
Subchapter 8G Section .0201 requires an owner of a classified water pollution control
system to designate one ORC and one or more Backup ORCs certified by the Water
Pollution Control System (WPCS) Operators Certification Commission (Commission) of
the appropriate type and grade for the treatment system. The rule also requires the owner
to submit a signed completed "Water Pollution Control System Operator Designation
Form" to the Commission countersigned by the designated certified operators within 120
calendar days following a vacancy in the position of ORC or Backup ORC. In the
Division's data system, the designated ORC for the Town was Leondies Parks. Mr.
Parks has retired and is no longer the designated ORC as of April 18, 2019. Please
complete the attached WPCS Operator Designation Form and suhnnit it to the Division.
12. Cleaning Sewer Lines. The Town has developed and implements a program for cleaning
the sewer lines. As required by Section I1.8 of the subject permit, at least 10% of the
wastewater collection system should be cleaned each year. Preventative cleaning is not
required for sewer lines less than five years old, unless an inspection reveals the need for
cleaning. The on -site representative indicated that 5,869 linear feet of sewer line was
cleaned during calendar year 2018.
13. Spill Response Action Plan. The Town has developed and maintains a Spill Response
Action Plan that includes 24-hour contact numbers, response time, equipment list and
spare parts inventory, access to cleaning equipment, access to construction crews, sources
of emergency funds, site sanitation and cleanup materials, and post-SSO assessment. A
copy of the Plan was submitted with the Town's permit renewal application package
received December 15, 2017.
14. SSO Records. The Town has maintained adequate records pertaining to SSOs for the
three-year period ending April 20I9. The last reportable SSO occurred on January 24,
20I9.
15. Comprehensive Sewer Map. The Town maintains an up-to-date comprehensive map of
its sewer collection system. The map includes pipe size, pipe Iocation, flow direction,
and each pump station identification, location and capacity. The To►vn will need to
update the ntap to include pipe material, pipe age and number of active service taps.

16. Inspection of High Priority Lines. During the state inspection, the following high
priority sewer lines were inspected: an aerial line and a sub -waterway crossing line at
Montgomery Street; an aerial line near the Country Inn Lift Station; and an aerial line at
the 581 North Lift Station. No compliance issues with the sewer lines were observed. As
required by Section VA of the subject permit, the permittee is required to inspect all high
priority sewer lines at least once every six -months. During 2018, the Town inspected the
high priority sewer lines on September 12th and October 23"d. At the time of the state's
inspection, the Town's records did not reflect any high priority sewer line inspections in
2019. Within 45-days of receiving this letter, please update this office regarding the
status of the Town's high priority lure inspection program. Please ensure all high
priority lines are inspected at least once every six months as required by the subject
permit.

Comment The audible alarm did not function when the high water float was activated. An inspection of the
logbook showed the station was ins ected less than weekly during February and April 2019.
The location of the visual alarm is low and is not visible from the road near the school. The
permit re uires the alarms to be placed in a clear and conspicuous location.
