Columbus Street Chapel Hill, NC 27514 Project Name: Burning Tree Drive Drainage Improvements Project On August 17, 2009, the 401 Oversight/Express Permitting Unit of the Division of Water Quality received a request from Ms. Cindy Carr, of Mulkey Engineers & Consultants, Inc., regarding a project known as "Burning Tree Drive Drainage Improvements Project" for acceptance into the Express Review Program. This letter advises you that your project will be accepted into the Express Review Program once the following items are received: 1) The application fee of $4,000.00 [for 401 Water Quality Certification with major impacts ($2,000) + Riparian Buffer Authorization ($2,000)], made payable to the North Carolina Division of Water Quality]; 2) Five (5) complete and collated copies of the PCN Application (instead of providing five copies of the full size plans you may provide two copies of the full size plans along with three copies of the 11X17); 3) One (1) copy of the completed PCN Submittal Checklist (httl2://h2o.enr.state.nc.us/ncwetlands/documents/PCNChecklist vl 4 doc); 4) Stormwater Management Plan (if applicable): a. Three (3) copies of all supporting information pertaining to this project including: i. Design calculation sheets for all proposed BMP(s) sized for both on-site and off-site drainage, if any; ii. BMP Supplement for each proposed BMP including Design Summary and Required Items Check List (available at http:Hh2o.enr.state.nc.us/su/bmp forms htm) along with all required items; iii. A signed and notarized Operation & Maintenance Agreement for each BMP; b. Three copies of full size plans (with detailed construction information) shall include the following: i. Entire site plan with existing and proposed contours and delineated drainage areas to the BMPs; ii. Erosion and sediment control plans; iii. Plan views and cross sectional details of all BMPs and associated structures with elevations; iv. Construction sequence and planting schedule for the proposed BMPs; 5) One (1) data CD of full size plans in TIFF Group 4 format (black and white, not grayscale or color). If the plans are too large to store in TIFF format, they can be stored in PDF. A pre-application meeting (to include the property owner) is required prior to the submittal of your completed application package.
