C. 28621 Dear_ Mr. Hadden: General Manager SUBJECT: Permit No. 14378 Weyerhaeuser Company Elkin Oriented Stand Board Plant Wastewater Treatment and Spray Irrigation Facilities Surry County In accordance with your application received December 10, 1.986, we are forwarding herewith Permit No. 14378, dated March 9, 1986, to Weyerhaeuser Company for the construction and operation of the subject wastewater collection system extensions.( This permit shall be effective from the date of issuance until March 31, 1992, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh North Carolina 27604. If you wish to have the hearing before the Hearing Officer with this Department, you must indicate in the petition that you waive the right to have the contested case conducted by a Hearing Officer in the office of Administrative Hearings, and wish to have the matter conducted in the Department of Natural Resources and Community Development. Ramsey Groundwater Section Simmons -Eastern Company NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Weyerhaeuser Company Surry County FOR THE construction and operation of a 14,300 GPD wastewater treatment and disposal system for Venturi scrubber blowdown consisting of a lined 420,000-gallon aerated lagoon, a 412,000-gallon lagoon for storage during inclement weather, a vertical chopper pump and 8 spray nozzles in four spray fields with. 1.0.4 acres of total wetted area, with no discharge to the surface waters of the State. pursuant to the application received December 10, 1986, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until March 31, 1992, and shall be subject to the following specified conditions and limitations: 1. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature and volume of waters described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4. This permit is not transferable. 5. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes, and may be rescinded unless the facilities are installed, maintained and operated in a manner which will protect the assigned water quality standards of the surface waters and ground waters. 6. In event the facilities fail to perform satisfactorily including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or disposal. facilities. 7. Solids, sludges, or other pollutants removed or resulting from the wastewater storage facilities shall be contained and disposed of in such a manner as to prevent any contamination of the surface or groundwater of the State. 8. The Permittee shall. employ a certified wastewater treatment plant operator in responsible charge of the wastewater treatment facilities. Such operator must hold a certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. 9. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. 1_0. Within 90 days of permit issuance, monitoring wells shall. be installed as specified below: (a) One (1) upgradient and one (1) downgradient of the spray irrigation field, (b) One (1) npgradient and three (3) downg.radient of the wastewater treatment lagoons. The location and construction details for these wells should be approved by the Winston. -Salem Regional Office, from which a well construction permit must be obtained. 11. The monitor wells must be sampled initially after construction and thereafter every March, July and November for the following parameters: NO3 Total Ammonia TDS SO4 Zn Cu TOX Water Levels pH TOC Chloride Phenol Orthophosphate Formaldehyde COD The measurement of water level must be made prior to sampling for the remaining parameters. 12. The applicant shall provide an analysis of the actual operational wastewater to the Groundwater Section, NC Division of Environmental Management within 60 days of permit issuance. This analysis should fully characterize the organic and inorganic constituents in the wastewater just prior to land application. 13. Any additional groundwater quality monitoring, as deemed necessary by the Department, shall be provided. 14. A suitable year round vegetative cover shall be maintained. 15. The issuance of this permit shall. not relieve Weyerhauser Company of the responsibility for damages to surface or ground waters resulting from the operation of this facility. 16. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater_ resulting from the operation of this facility. 17. No type of wastewater other than that from Weyerhauser Company shall be sprayed onto the irrigation area. 18. Adequate facilities shall be provided to prevent surface water runoff from entering the spray field. 1.9. Adequate inspection, maintenance, and cleaning shall be provided by the permittee to insure proper operation of the subject facilities. o 20. The applicant rates shall not exceed: a. 0.3 inches/hour b. 1.0 inch/acre/week 21.. The follower buffer distances must be maintained from the wetted perimeter: a. 150 ft. from property lines; b. 50 ft. from a dry weather ditch or drainage area; C. 100 ft. from a flowing stream; d. 100 ft. from a well; e. 400 ft. from a residence. 22. No spraying shall be done in low-lying areas or areas condutive to ponding. 23. Wastewater shall. not be applied during inclement weather or until 24 hours following a rainfall event of 1/2-inch or greater in 24-hours, or when the ground is frozen and in a condition which may result in surface runoff. 24. Wastewater shall be uniformily applied as to prevent any surface water runoff, ponding, or erosion. 25. The Winston-Salem Regional office, Phone No. 919/761-2351, shall be notified at least twenty-four (24) hours in advance of operation of the installed disposal system so that an in -place inspection can be made. Such notification to the Regional. Supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Fri -day, excluding State Holidays. 26. The Permittee, at least six (6) months prior to the expiration of this Permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if indicated, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate.
