ROCKINGHAM COUNTY SCHOOLS May 15, 2019 TO WHOM IT MAY CONCERN: MCEIVED/NCIDEWDWR JUN 0 3 2019 water (duality Permitting Section We received a "Notice of Violation and Assessment of Civil Penalty" for Bethany Wastewater Plant (NC0037001). When our second sample came back high, we changed the bulbs in the UV system before requesting assistance from NCDEQ. We also purchased an additional two sets of UV bulbs (see attached invoice). On March 13, 2019, we met with Gary Hudson to discuss higher than normal fecal coliform results as shown on our March 2019 eDMR report. Per Mr. Hudson's recommendation, we disassembled the UV system and cleaned the protective sleeves. We have implemented a monthly cleaning schedule for the protective sleeves at the plant. After investigation of "What Changed" in our process. We discovered a possible cause. In September 2018, we changed cleaning/sanitation products to a product called Envirox (see attached SDS). I also attached a summary of Pace Analytical Fecal Coliform results. We stopped using Envirox on May 14, 2019 at that site. We are not yet sure that there is a connection. This assessment is based upon the following facts: a review has been conducted of the Discharge Monitoring Report (DMR) submitted by Rockingham County Board of Education for the month of February 2019. This review has shown the subject facility to be in violation of the discharge limitations and/or monitoring requirements found in NPDES WW Permit No. NC0037001. The violations, which occurred in February 2019, are summarized in Attachment A to this letter. Based upon the above facts, I conclude as a matter of law that Rockingham County Board of Education violated the terms, conditions or requirements of NPDES WW Permit No. NCO037001 and G.S. 143-215. 1 (a)(6) in the manner and extent shown in Attachment A. Although nothing conclusive was found to be the cause of the fecal coliform problems, the following remedial actions were discussed: o When you took over as Operator in Responsible Charge (ORC), you were unaware that ultraviolet radiation disinfection systems require the regular dismantling and cleaning of the protective sleeves on a frequent schedule. It would be helpful to check with the manufacturer for all suggested maintenance schedules. o When you did decide to take the unit down and clean the sleeves you noticed an immediate improvement in your fecal coliform sample results.
