This letter is in reference to your permit renewal request received May 24, 2019.
During the pre -review process, it was noted that the following items were missing from the
submitted application package:
1. Application Form: RLAP 06-16 was not included. This form may be downloaded at:
https:Hdeg.nc. gov/about/divisions/water-resources/water-resources-permits/wastewater-
branch/non-discharge-permittin -ug; nit/application.
2. Per Instruction E of Form: RLAP 06-16, an Operation and Maintenance Plan for renewal
application packages shall be submitted, which shall include:
a. Operational functions; describe the operation of the program to show what operations are
necessary for the program to function and by whom the functions are to be conducted.
b. Maintenance schedules; may include equipment calibration; maintenance of signs, etc.
c. Safety measures; may include safety training program, manuals, signs, etc.
d. Spill response plan; including control, containment, remediation, emergency contact
information, etc.
e. Inspection plan including the following information;
f. Names and titles of personnel responsible for conducting the inspections.
g. Frequency and location of inspections, including those to be conducted by the ORC, and
procedures to assure that the selected location(s) and inspection frequency are
representative of the residuals management program.
h. Detailed description of inspection procedures including record keeping and actions to be
taken by the inspector in the event that noncompliance is observed.
