Part D 5 — the application requires the E&SC approval documentation (such as a letter), not the E&SC Plan that was
uploaded instead (attached). The approval letter or other documentation should include the E&SC Project or Permit
number, the approved acres to be disturbed, and the approval date.
Part D 6 — the NOI Certification Form was not dated (see attached).
