III. TOTAL ACRES AVAILABLE TO RECEIVE WASTE DURING 5 DAY DRAW DOWN PERIOD'a
'While this section deals with hydraulic loading capacities, applications cannot exceed
agronomic rate for receiving crop according to its certified waste plan.
2Fields with no remaining PAN balance, no receiving crop, and/or completely saturated are
not considered available to receive waste.

IV. FACILITY'S PoA OVERALL HYDRAULIC LOAD TO BE LAND APPLIED PER ACRE
PoA l Day) 2/ Ii2'{7S17o- 0.62 Inches per acre to be applied within 5 days
line x
If unable to land apply hydraulic load listed in line y, list course of action here including pump
and haul, depopulation, herd reduction, etc. For pump $ haul and herd reduction options,
1. Describe moisture conditions of flelds? (e.g. Is there water standing in field; does
2. Date and amount of last rainfall event?
DATE:=-65129118t AMOUNT: F 3.50 Inches
3. Dates of last waste application event per field:
June-0 20.1137,-::l
4. Given optimum soil and weather conditions, Is irrigation equipment capable of applying the volume In

r=rai
Shorter pumping time and lower application rates will be used. Smaller o rings and faster travel speeds will be
used. Aeroway system will be•used when soil conditions improve.
Robert Marasco called on 5-28-18 to report freeboard. Determined on 5-30-18 that the freeboard was at 11"
and needed a 5 day POA. Revised 30 dayPOA to 5 day POA on 6-1-18. This information was provided by
Robert Marasco on 6-1-18. Completed by Jason Turner PSWCD on 6-1-18.
Requested update by noon on 6-5-18, but got it. Received call from Robert 6-6-18 about revising 5 day POA.
Received updated Information this morning 6-6-18'and revised on 6-6-18. New information provided on 6-11-18.
