SFR Single -Family Residence Wastewater Irrigation System Forsyth County Division of Water Resources' Central and Regional staff has reviewed the application package received February 6, 2019 and additional information received April 23, 2019. However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on June 24, 2019. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through D, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Soil Evaluation: 1. The Licensed Soil Scientist indicated in his letter that the Bt soil horizon was the only horizon measured because it was the least restrictive horizon. In accordance with 15A NCAC 02T .0604(b)(2), hydraulic loading rates shall be based on in -situ measurements of saturated hydraulic conductivity in the most restrictive horizon. Provide a clarification on whether the Bt soil horizon was the most or least restrictive. 2. Upon a second site inspection, a drainage feature was found within the 25 foot setback of the proposed spray field. A field delineated detailed soils map shall be submitted, identifying an updated suitable irrigation area with this feature included. This map should be signed, sealed and stamped by a North Carolina Licensed Soil Scientist. B. Eneineerine Plans: 1. See A.2. The spray field plans need to be updated with the changes in suitable irrigation area. The current spray heads affected by this change are spray heads #3 and #4. Once the spray field's configuration is redesigned, the area must be calculated to verify that it meets the minimum of 14,950 square feet needed from the SFR Loading Rate Workbook (SFRLRW). See comment D.1. 2. The Overall Site Map on Page 2 of 6 of the construction plans indicates that spray heads #3, #4, #6 and #7 have partial spray radii with wetted diameters that don't extend radially outward from the spray head. The wetted diameter must always extend in a straight line outwards from the spray head. Please adjust the drawings that show the corrected wetted zones and make any adjustments to the field area in the application and engineering calculations. 3. During the site inspection, it was noted that the driveway that was shown on the Overall Site Map page of the plans is incorrect. It appeared that the driveway followed the old road bed that runs adjacent to the field. As the irrigation area must be restricted from public access by a minimum of a two strand fence, the site map must be revised to show the new driveway location in respect to the fencing. C. Engineering Calculations: Per 15A NCAC 02T .0604(c)(3), provide hydraulic and pollutant loading calculations for each treatment unit demonstrating how the designed effluent concentrations in Application Item V.I. were determined (Note: "black box" calculations are unacceptable). D. Recommendations (Response not required): 1. The attached sketch details the changes that the regional inspector suggests based on conversations with Chris Murray, the soil scientist consultant. 2. It was noted that the permittee had begun to grade the driveway into the proposed home site following the old road bed mentioned in comment A.3. Please note that any cut of two feet or more in vertical height that may arise from this grading has an associated 15 foot setback to the irrigation area as per 15A NCAC 02T .0606. Because the old road bed was seen to be adjacent to the irrigation area, it is worth mentioning that some areas currently proposed for irrigation will become unsuitable Mr. Andrew Miller May 23, 2019 Page 3 of 3 due to the 15 foot setback if additional grading occurs close to the irrigation area with cuts of greater than two feet. 3. The regional inspector noticed that the spray heads are currently proposed at 18 inch above grade. Due to the slope present in the spray field, it is recommended spray heads be installed to 24-36 inches. 4. The water balance of the system from the SFRLRW suggests that the minimum of 5 days of effective storage would be insufficient.
