On October 18, 2006, the NC Division of Water Quality received your application for
stream culverting with respect to your proposed development along John Mason Road.

application was incomplete and/or provided inaccurate information as discussed below.

impact protected wetlands and/or streams on the subject property. Therefore, unless
we receive the additional information requested below, we will have to move toward
denial of your application as required by 15A NCAC 2H .0506 and will place this project
on-bald as incomplete-untif-we receive this additional-infomratierr.~-Please provide the

15A NCAC 02H .0506 (f) requires that "a lack of practical alternatives" be
demonstrated. No discussion in regards to developing the project without the
impacts requested has been presented. A discussion regarding practical
alternatives with respect to a reduction of impacts must be presented.

Application Item VI. 4. requests a list of all impacts. In your submitted list, you
indicate that 50 linear feet of impact is required. As these two figures do not
agree, you application must be modified to reflect the true amount of impacts.
3. Application Item III. 10. requests a description of the overall project. Your
submitted application details the requested impact, but fails to describe the
proposed use of the 200.098 acres. You must review the instructions for
Application item III. and provide not only site plans, but proposed building sites, a
delineation of all streams and wetlands within the project boundary, and land use
descriptions.
4. Application Item XI. Requests a stormwater management description for the
entire development, for the life of the development. You application states that
the land will be left undisturbed, negating the need for access. Please clarify the
need and purpose for the crossing, and if for development, provide a detailed
description as to how stormwater will be managed for the life of the development.
5. Application Item XII. Request a description as to how sewage will be disposed of.
As you have stated in your application that the proposed use id residential, you
must provide a description of how wastewater will be managed and disposed of.
N/A is an unacceptable answer.

information to both the 401 Permitting Unit at:
