received on June 7, 2005. After reviewing your application, we have determined that this project is covered by
Water Quality General Certification Number 3492, which can be viewed on our web site at

198200030 once it is issued to you by the U.S. Army Corps of Engineers (USACOE). This letter also serves

It was brought to our attention that a signature was needed for our forms and I don't
know what form needs to be signed. If this does not suffice for a signature, please fax the

There was also some question about the square footage of the project. The retaining wall
is about 70 feet across and 4 feet tall, the land behind the retaining wall that will be
disturbed, or filled in is an area that is 20 feet from waters edge landward, and about 80
feet from end to end for a total of about 1600 square feet.

Thank you for taking the time to speak with me on June 2nd, regarding our retaining wall
and rip rap at 100 Wickford Rd I spoke with our contractor, Brooke Foxcroft of Fox
Docks after speaking with you and he provided me with the cncloscd documents,
indicating he had submitted the application within the past two weeks. He also said he
sent in the $200.00 application fee.

If you could please vcrify this for me. If the application fce has not bcen sent in, I will
scnd it to you immcdiately upon hearing from you. I am also enclosing a few pictures
that we have of both before and after of the property.

We contracted with the contractor on or about February 14,2005 and he infom1ed us the
project was finished around the beginning of May. He told us he normally does these
projccts while the papcrwork is being processed, because it takes about the same amount
of time to process the papcrwork as it does to do the stabilization, and as long as Duke
Power doesn't see any issues, there usually aren't any.

Thank you for taking thc timc to speak with me on June 2nd, regarding our retaining wall
and rip rap at 100 Wickford Rd I spoke with our contractor, Brooke Foxcroft of Fox
Docks after speaking with you and he provided me with the enclosed documents,
indicating he had submitted the application within the past two weeks. He also said he
sent in the $200.00 application fee.

If you could please verify this for me. Ifthc application fee has not been sent in, I will
send it to you immediately upon hearing from you. I am also enclosing a fcw pictures
that we have of both beforc and after of the property.

We contracted with the contractor on or about February 14,2005 and he informed us the
project was finished around thc beginning of May. He told us he normally does these
projects while the paperwork is being processed, because it takes about the same amount
of time to process thc paperwork as it does to do the stabilization, and as long as Duke
Power doesn't sec any issues, therc usually arcn't any.

any subsequent modifications, the applicant is required to return this certificate to the 401/Wetlands Unit,
North Carolina Division of Water Quality, 1621 Mail Service Center, Raleigh, NC, 27699-1621. This form
may be returned to DWQ by the applicant, tbe applicant's authorized agent, or the project engineer, It is
not necessary to send certificates from all of these.
