On May 21, 2019, staff ofthe NC Division of Water Resources (DWR), Water Quality Regional Operations
Section (WQROS), inspected the subject single-family residence surface irrigation wastewater treatment
and disposal system. The purpose of the visit was to conduct a compliance inspection.
On the day of the inspection all single-family residence wastewater treatment and application equipment
appeared to be well maintained. Tanks and floats were not checked (concrete lids) but appeared to be
operating properly. The spray field was in good condition all heads were operating properly.
I have included a partially completed, Non -Discharge System Renewal Form and the Operation and
Maintenance Agreement for your convenience. Your permit expires November 30, 2019. Please look over
the forms and correct or add any infonnation necessary, return the completed forms to the address on the

Inspection Summary
