On August 12, 2009,the Division of Water Quality(DWQ)received your application dated March 11,
2009 for the above referenced project. The DWQ has determined that your application was incomplete
and/or provided inaccurate information as discussed below. Please provide the following so that we may
continue to review your project.
Additional Information Requested:
1. The 401 Certification cannot be processed until five(5)complete sets of the application are
received at the DWQ Central Office in Raleigh. Please submit three(3)more application sets to
begin the review and approval process. DWQ acknowledges that a full application set was sent to
Wilmington Regional Office to Ms. Joanne Steenhuis
Until the information requested in this letter is provided, I will request(by copy of this letter)that the
Corps of Engineers place this project on hold. Also,this project will be placed on hold for our processing
due to incomplete information (15A NCAC 2H .0507(a)).
