This Review, undertaken in accordance with G.S. 143-215.10D, is one of two visits scheduled for all registered livestock operations during the 1999 calendar year. The Division of Water Quality will conduct a second site inspection. During the Review, it was noted that the small grain over -seeded on bermuda had been over-apppplied greater than 10% of the PAN recommendation for this crop. According ion to Sect111.6.F. of the general permit issued by the Division of Water Quality, it states that overapplying animaigwaste either in excess of the limits set out in the CAWMP or where runoff enters surface waters should be reported to the Division of Water Quality. Over -application occurred on hydrant #10 by 19 lbs. of nitrogen to small grain over - seeded to bermuda. You are strongly encouraged to contactour certified technical specialist for help. You may also want to contact your local Soil andyWater Conservation District Office since they may be able to provide you with additional technical and/or financial assistance to implement corrective best management practices. Please remember that in order for your facility to be in compliance with environmental regulations, animal waste cannot be discharged into the waters of the State, and the animal waste collection, trealment, storage and disposals stems must be properly sized, maintained and operated under the responsible charge ofYa certified operator. Notice of Referral will be sent to Division of Water Quality in regards to overapplication. 17- Need to have Certificate of Coverage (2 page letter) and General Permit for Swine Operation with paperwork on farm site. 18- Need a copy of Waste Utilization Plan with paperwork on farm site. Need to fill out insect control, odor control, mortality checklist, and emergency action plan for farm site. Need a copy of all storage calculations of all lagoons with paperwork on farm site from technical specialist which certified the facility. 19- Need to contact Enviro-Ag Compliance in regards to record keeping. Crop types need to correspond with crops in field was Utilization Plan. Crops were incorrect on Irr-2 forms, no com crop on farm site. Need to have updated irrigation records r. Farm is Permitted. 26- Need to extend discharge pipes out into lagoon pool area so waste is discharging at or below lagoon level surface. 32- Need to extend fill pipes on flush tanks and pull pit boxes to the bottom of tanks/boxes for odor control measures. t 3123199
