I hope this finds you well. Please see responses to your inquiries below.
• 1 don't see a map or plan sheet that clearly shows where the temporary crossing is to be located. I see
on the plan sheets that there's a hall road called out and a bridge stream crossing, is that where it is now
proposed to be culvert crossing? If so, has a modification to the erosion control plan been secured (or
applied for)?
The proposed temporary stream crossing is to replace the temporary bridge shown on drawing C-13
Erosion & Sedimentation Control Plan — Phase 3. We are seeking a determination on permitting the
disturbance to the stream and wetlands before filing a modification to the erosion control plan.
You provide a justification for the impacts but it doesn't include why a temporary bridge is not a
practical option.
The contractor is concerned that it will be difficult to keep the temporary bridge in place when
scraper/pans loaded with 100,000 pounds of fill dirt descend down the steep slope from the borrow
site and hit the edge of the bridge
• It is not clear from your plans that the culvert installation will be installed "in the dry'. Can you please
provide documentation of pump around, increase the temporary impact length as necessary and
provide a pump around specification.
We are seeking a determination on the permitting requirements for the temporary disturbance of the
stream and wetlands. If approved we will prepare plans to pump around the temporary stream
crossing so it can be installed "in the dry'. Please note that the culvert length has been increased to
28'
• The plan sheet specifies, "see stream restoration detail" for culvert removal but I couldn't find a stream
restoration detail.
The detail is added to the attached plan. It is attached to this email as a PDF.
• Our standard requirement in the General Certification is for pipes to be buried for aquatic passage. If
you want to request an exception please state that and provide a justification for the request.
The plans show the temporary pipes partially buried in the stream bed in Cross Section A -A. The depth
now complies with the standard.
• Has the borrow site been delineated for streams/wetlands? Can you please provide a map that shows
the limits of the borrow site.
Streams and wetlands were not identified on the neighboring site. Excavation of the site was started
years ago by the land owner. The current excavation is permitted as an extension of that
project. Additionally, the project is funded by the Cooperative Finance Corporation. Attached
find an approved sedimentation and erosion plan for the site (showing that it is a different
project).

This office has reviewed the subject sedimentation and erosion control plan. We find the
pIan to be acceptable with following conditions and hereby issue this letter of approval.
This plan approval shall expire three (3) years following the date of approval, if no land -
disturbing activity has been undertaken, as is required by Title 15A NCAC 413.0029.
Be advised that Title 15 NCAC 4B.0017(a) requires that a copy of the approved erosion
control plan be on file at the job site. Also, you should consider this letter to give the
Notice requi
Nred by GS 113A -61(d) of our right of periodic inspection to insure
compliance with the approved plan.
North Carolina's Sedimentation Pollution Control Program is performance oriented,
requiring protection of the natural resources and adjoining properties. If following
commencement of this project it is determined that the plan is inadequate to meet the
requirements of the Sedimentation and Pollution Control Act of 1973 (North Carolina
General Statute 113A-51 through 66), this office may require revisions to the plan and its
implementation of the revisions to insure compliance with the Act.
Acceptance and approval of this plan is conditioned upon your compliance
with Federal State water quality laws, regulations, and rules. In addition,
local city or county ordinances or rules may also apply to this land -
disturbing activity. This approval does not supersede any other permit or
approval.
Please note that this approval is based in part
on the accuracy of the information provided
in the Financial Responsibility Form which you have provided. You are requested to file
an amended form if there is any change in the information included on the form. In
addition, it would be helpful if you notify this office of the proposed starting date for this
project. Please notify us when you plan to have a preconstruction conference, and we
will plan to attend.
Please be aware that if more than one acre is to be disturbed by this project, you will
be covered by the enclosed general storm water permit NCGO1000 (Construction
Activities).
