However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on June 17, 2019. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through B, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. All revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper copies and one electronic copy submitted to my attention at the address below. Site May: 1. The site map provided in the permit renewal submission needs a couple of revisions, including: ➢ The review and compliance boundary are not drawn correctly. The compliance boundary cannot cross the property owned by CSX Transportation (the railroad). Please refer to the attached map to assist you when revising your review and compliance boundary. ➢ The map provided includes ditches, waterways, and surface water swales. The ditches were buffered 100 feet, but the surface water swales and waterways were buffered 25 feet. The following buffers are the correct buffers to use for the wetted area of this wastewater irrigation system: i. Each habitable residence or place of assembly under separate ownership: 400 ' ii. Each private or public water supply source: 100 iii. Surface waters: 100 iv. Groundwater lowering ditches: 100 v. Surface water diversions (upslope): 100 vi. Surface water diversions (downslope): 100 vii. Each well with exception of monitoring wells: 100 viii. Each property line: 150 2 ix. Top of slope of embankments or cuts of two feet or more in vertical height: 15 x. Each water line: 10 xi. Each swimming pool: 100 xii. Public right of way: 50 xiii. Nitrification field: 20 xiv. Each building foundation or basement: 15 ' Habitable residences or places of assembly under separate ownership constructed after the facilities herein were originally permitted or subsequently modified are exempt from this setback. Z Setbacks to property lines are not applicable when the Permittee, or the entity from which the Permittee is leasing, owns both parcels separated by the property line. Determine if the surface water swales and waterways are surface water diversions, surface waters, or not a feature that requires a buffer. If they are surface water diversions or surface waters, a 100 foot setback will be required for these features as shown on the attached map with the hatched area. Modify the wetted area and Waste Utilization Plan to accommodate these buffers, if necessary. B. Waste Utilization Plan 1. Provide a copy of the most up-to-date Waste Utilization Plan for this facility.
