page of application. Agent's signature is valid only if an authorization letter from the
applicant is provided. Please sign and date.
® The 401 Certification cannot be processed until five (5) complete sets of the application and
associated maps is received at the DWQ Central Office in Raleigh along with the appropriate
fee.
Please submit this information within 30 calendar days of the date of this letter. This letter only
addresses the application review and does not authorize any impacts to wetlands, waters or
protected buffers. Please be aware that any impacts requested within your application are not
