Part D #5 - the uploaded document was an email informing you to obtain coverage under the NCG01. We need an
approval letter from the entity that reviewed your E&SC plan.
Part D #6 - the paper NOI Certification Form needs to list Summit Pointe Partners, LLC as the Legally Responsible
Organizational Entity because that's the entity listed in Parts B & E.
