for the Person County Human Services Facility. Within the approval letter, there is a condition that reads:

The applicant and/or authorized agent shall contact the DWQ Express Review Program in writing at the
letterhead address within 10 days of the commencement of construction.

This is a part oftt1e Express Review Program that is required to ensure compliance during construction and post
construction. The~efore, if you have begun construction on your property, you are required to notify this office in
writing by July 24,2006. If you do not notify this office in writing and construction has started, you are in violation
of the Water Quality Certification Rules as required by ISA NCAC 2H .0507(c).

The Division1 of Water Quality (DWQ) has reviewed the Sto~ Water Management Plan prepared by your
engineer, Mr~ Steven Bailey, P.E. of Coulter, Jewell, Thames, FA. This Plan satisfies Condition 6 of the
Isolated Wetlands Permit issued on March 15, 2005 (DWQ Proj~ct No. 05-0216) and modified on April

The structural storm water practices as approved by this Office as well as drainage patterns must be
maintained irt perpetuity. No changes to the structural storm water practices shall be made without
written authorization from the Division of Water Quality. The storm water easements shall allow for the
ability to maihtain the structures, perform corrective actions as described above and shall provide
protection of the structures from potential alternations by future property owners.
One of the c~nditions of the Express Review Program is that the applicant and/or authorized agent shall
contact the DWQ Express Review Program in writing at the letterhead address within 10 days of the

Dear Mr. Bailey:
On March 30, 2005, the Division of Water Quality (DWQ) received your Express Review Program Stormwater
Management Application dated March 29, 2005. The DWQ has determined that your application was incomplete
and/or provided inaccurate information as discussed below. The DWQ will require additional information in order to
process your application to impact protected wetland, streams and/or buffers on the subject property. Please provide
the following information so that we may continue to review your project. .

1. Stormwater Management must be provided for the entire site (reference Condition 4, Bullet Point 2 of GC 3402,

2. Please re-draft the Operation and Maintenance Agreement to include maintenance of all storm water management
facilities on the site! .

is a process that requires all parties to participate in a timely manner.

Application along with a check for the submittal fee of $2,000.00 for the Person County Human
Services Facility.

The Stormwater Plan is required as part of an "APPROVAL ofIsolated Wetlands Permit" by the
Division of\\{~ter Quality for the project dated March 15,2005. The project ptoposed to fill a portion
of wetlands t9 expand the available parking to meet local requirements. The Stormwater Plan includes
an undergrouqd sand filter to treat a portion of an existing parking lot that is larger than the parking
addition causirg the wetlands fill. !

I SAND FILTER OPERATION AND MAINTENANCE AGREEMENT

The sand filter system is defined as the sand filter structure.
I
Maintenance activities shall be performed as follows:
1. At least twice pJr year, after a significant runoff producing rainfall event:
a. Inspect the s~nd filter to determine if the filter bed is passing the runoff as expected. Maintenance
must be perf?rmed when storms of approximately one inch are not passing through the filter within 24
hours. Maintenance for the filter bed will consist of removing the first two or three inches of discolored
sand and replacing the new sand.

b. Inspect the sediment chamber for sediment accumulation. The sediment chamber will be pumped and
cleaned once sediment has accumulated to a depth of one foot. i
2. Inspect and rep~ir the collection system (Le. catch basins, piping, swales, ribrap, etc.) quarterly to
maintain proper tunctioning. :
3. Check and clea1 ~he overflow weir of any obstructions after all significant runoff producing rainfall events.
4. All components of the sand filter system shall be maintained in good working order.

General
New Plan Excel Realty proposes to fill 0.277 acres of isolated wetlands to facilitate
parking lot construction at the Person County Human Services Facility. The facility was
previou~ly a Wal-Mart store, and is being renovated to provide lease office space for
Person G:ounty. The proposed parking lot is required to meet the parking requirements of
the City:ofRoxboro Zoning Ordinance. The Division of Water Quality issued an
"APPROVAL of Isolated Wetlands Permit" for the project on March; 15, 2005, as part of
DWQ PfoieC1 #05-0216. .
An underground storm water sand filter will be constructed to treat storm water runoff
from a portion of the existing parking lot on the site. It is not feasibltf to construct the
sand filth to treat runoff from the proposed parking lot. An alternate location in the
existing [parking lot was selected for the sand filter installation. The impervious area of
the proppsed parking lot causing the wetland fill is 0.317 acres. The drainage area to the
propose~ sand filter is 0.388 acres, of which 0.379 acres is impervious. Therefore, the
proposed sand filter will treat more impervious surface than is being created by the
proposed parking lot causing the wetlands fill. '
Design I
I
The san~ filter has been designed based on guidelines included in the April 1999 revision
ofthe "Storm Water Best Management Practices" by NCDENR. The design
requirenlents listed in the BMP manual for both the sediment chamber and sand filter
chambet are a minimum volume of 540 cubic feet per acre, minimum depth of 18", and
minimt surface area of 360 square feet per acre.
There isla drainage area of 0.388 acres to the filter. The required and provided design
parameters are included in the tables below.

The sand, filter will be a pre-cast concrete structure. A concrete wall will divide the
structurcl into a sediment chamber and a sand filter chamber. At one end of the structure
will be ~ clear well area, sectioned off by concrete divider walls. Stormwater runoff from

the park~g lot will enter the structure through a traffic bearing over a section of the
sediment ,chamber. Ports through the wall separating the sediment chamber from the
sand filter chamber are located along the length of the structure. The ports allow
stormwater to flow into the sand filter chamber along the length of the structure. A
bypass ~eir is located between the sediment chamber and the clear well to allow large
storms t9:bypass the sand filter chamber. A geotextile fabric is placed beneath the sand
bed, on tpp of a layer of washed stone. Within the washed stone are three perforated
pipes to r treated stormwater to the clear well.
Maintenance

The sanqfilter must be inspected at least twice a year after a storm to. determine if the
filter beq is passing the runoff as expected. Maintenance should be performed when
storms o~ approximately one inch are passing through the filter with in 24 hours.
Maintenfpce for the filter consists of removing the first 2 or 3 inches of discolored sand
and replacing it with new sand. The sediment chamber should be inspected at the same
time as the sand filter bed. The sediment chamber shall be pumped and cleaned once

sedIment has accumulated to a depth of one foot.
The ovekow weir is to be inspected and cleared of obstructions after all significant
runoff Pfoducing events.
