Thank you.... I am going to fil out the rescission form for the NCG130047 permit and complete the
renewal COC for the NCG130019 permit. I will also update the information on line for the NCG130019 Permit
and Pay the annual $100 fee.

Please see the attached report. Can you confirm that this is the same site? It seems likely. If so, you will need
to submit a Rescission Form request for one of them so we can remove the duplicate. It should go to Suzanne
McCoy's attention.
Also, it looks no one ever requested a renewal COC for either one of these. That means you don't have any
record of the renewal. We sent out letters about that last year — attached is the one we sent for
NCG130019. After you decide which permit this facility will keep, please request a renewal COC on-line here. It
will be emailed to the requester:

The Durham facility has 2 NCG13 permits, both have fees due:
NCG130019 and
NCG130047
Is there a reason there would be 2? Can rescind one and keep the other?
