Dear Mr. Einsmann:
On March 07, 2019, the Division of Energy, Mineral and I,and Resources received your request to rescind
your coverage under NPDES Stormwater Permit NCG080784.
Based on our review of the facility on March 26, 2019 by our Washington Regional Office, your request
for rescission has been denied. Your request is denied due to remaining trash/illegal dumping and
previous spills to be abated on the site which are presenting potential stormwater impacts. A copy of the
Inspection Report is attached for your records.
Once the potential stormwater impacts have been removed you may re -apply for a rescission. Your
Certificate of Coverage Number NCGO80784 will remain in effect, and you must continue to comply with
your NPDES stormwater permit.

Inspection Summary:
Tho last documontod situ visit war, 2014, whon tho eito was denied a rescission request noting conditions had not been
cleaned up as outlined/required during a 2013 rescission site inspection.
I met Matt Einsmann, PE, Republic Services Environmental Manager onsite. He and another Replubic representative stated
that the company was preparing to sell the property. I informaed that all trash/ illegal dumping, any previous spills, etc. were
to be properly abated, so that no known onsite problems were left behind for any potential buyers to acquire, or to be later
surprised by.
Mr. Einsmann said that he would recontact me for another visit, as soon as the site has been properly cleaned/prepped in
order to be placed on the sales market. Mr. Einsmann stated the site will be properly prepared such that the NPDES SW
Permit can be rescinded.
