E M, Triple K Farm 4 697 Van Dorp Road Pantego, North Carolina 27860 SUBJECT: Operation Review Summary and Corrective Action Recommendations Triple K Farm Facility No. 07-24 Beaufort County Dear Mr. Peele: On June 12'�, an operation Review was conducted at the Triple K Farm swine operation in Beaufort County. This Review, undertaken in accordance with G.S. 143-215.10D, is one of two visits scheduled for all registered livestock operations during the 1997 calendar year, The Division of Water Quality will conduct a second site inspection. During the Review, it was determined that waste was not being discharged to the waters of the State, and the animal waste collection, treatment, storage and disposal systems of the farm was maintained and operated under the responsible charge of a certified operator. A copy of the completed review form is enclosed for your information. The following observances, questions, and management deficiencies were discovered and noted for corrective action or response ■ The facility's Waste Utilization Plan needs to be revised to show current operational activity and waste application receiving crops. It was noted during the Review, that the farm is serving as a gilt isolation unit for Benson Farms. Your Technical Specialist can assist you with the revision, • Contact your Technical Specialist to locate any perennial streams which may be on your farm. Follow applicable setback requirements. ■ Mow and shrub lagoon dike walls in accordance with the waste system operation and Maintenance Plan. • Stormwater from a portion of the facility is presently routed to the primary lagoon. Contact your Technical Specialist to discuss methods for diverting this flow away from all waste handling structures. • Annual soils sampling for the 1997 calendar year has yet to be conducted. ■ A lagoon waste analysis is required within 60 days of any land application event. Samples should be pulled as soon as possible. ■ Waste application records need to be maintained for all waste applied with a honeywagon. These records should follow the format suggested in the Operator's Training Manual (SLD 1 and SLD2 forms). • Proper markers will have to be installed in the lagoons prior to certification. 943 Washington Square Mali, Washington, Forth Carolina 27889 Telephone 919/946-6481 FAX 919/975-3716 An Equal Opportunity Affirmative Action Employer Mr. Clark Peele June 20, 1997 Page 2 Though not required at the time of the review, the farm will need to complete an odor, insect, and mortality checklist as well as an emergency action plan in preparation for obtaining a general permit. Your Technical Specialist can assist you in completing these items. Once the facility obtains a certified plan,.the following items should be maintained on site: completed certification form; Waste Utilization Plan (WUP); Operation and Maintenance (O&M) Plan for the waste management system; site schematic; any design survey materials which were prepared prior to construction; Site Evaluation (Form MRCS NC-CPA-17) and Hazard Classification (if applicable); measurements and calculated volumes for the waste structures; waste analyses; soils analyses During ensuing reviews, deficiencies noted above will be re-examined to determine if corrective actions were implemented. As discussed above, all registered livestock operations are required to obtain and implement an approved animal waste management plan by the end of this year. The plan must be certified by either a designated technical specialist or a professional engineer. For additional assistance with the plan, please contact your local Soil and Water Conservation District Office, the local Cooperative Extension Service Office, or a regional agronomist with the North Carolina Department of Agriculture. Facility: Triple K Farm Facility ID#: 7-24 Beaufort County Dear Mr. Peele: RECEIVED WASHINGTC)N 0FF1C>E NOV 19 1996 Senate Bill 1217, An Act to Implement Recommendations of the Blue Ribbon Study Commission on Agricultural Waste, enacted by the 1996 North Carolina General Assembly, requires a certified operator for each animal waste management system that serves 250 or more swine by January 1, 1997. The owner of each animal waste management system must submit a designation form to the Technical Assistance and Certification Group which designates an Operator in Charge and is countersigned by the certified operator. The enclosed form must be submitted by January 1, 1997 for all facilities in operation as of that date. Failure to designate a certified operator for your animal waste management system is a violation of 15A NCAC 2H .0224 and may result in the assessment of a civil penalty. If you have questions concerning operator training or examinations for certification, please contact your local North Carolina Cooperative Extension Service agent or our office. Examinations have been offered on an on -going basis in many counties throughout the state for the past several months and will continue to be offered through December 31, 1996.
