Dear Mr. Czar:
Mitch Hayes of the Raleigh Regional Office conducted the subject inspection on May 07, 2019.
The assistance and help provided by Gerald Cox, Kay Bunnell, Jason Satterfield, and David
Rivera with this inspection was greatly appreciated. Below is a list of findings developed from the
inspection and review of the permit.
1. The subject permit was reissued, became effective December 01, 2016 and will expire
November 30, 2024.
2. The City of Sanford operates and maintains a Class-3 collection system. Basinwide
Information Management Systems (RIMS) lists Gerald Cox as the CS grade-3 Operator in
Charge (ORC) and Kristopher Furmage as the CS grade-3 back-up ORC.
3. A map of the collection system is being maintained on paper and on computer. The map
contains all required permit information,
4. The Capital Improvement Program was updated this year and includes projects for fiscal
years 2016 through 2024. Known deficiencies have been identified and are prioritized by
importance. Monies are approved yearly for projects_
5. Education materials on proper grease disposal are sent to customers twice a year.
Information on proper grease disposal is also available on the web. Food establishments
are inspected throughout the year. There have been no enforcements for the past year.
6. Work orders, complaints, repairs made to equipment, line cleaning, right of way
maintenance, high priority line inspection, SSO's, pump station maintenance are
maintained in separate log books. Log books are reviewed monthly.
7. According to the permit, the amount of gravity sewer in the collection system is 207 miles
or 1,092,960 feet. A review of the records indicate that 190,674 feet have been cleaned
this year.

S. BIMS lists 27 Sanitary Sewer Overflows (SSO's) for the review period January 2016 —
April 29, 2019, Three SSO's resulted in NOV's being issued, Documentation of the SSO's
and SSO's that are below reporting thresholds are being kept.
9. There are 9 duplex pump stations in the collection system. All pump stations have
restricted access, contact information signage, audible 1 visual alarms, on site generators,
and SCADA. All pump stations are visited daily. Pump stations inspected were Gasters
Creek, Lemon Springs, Kelly Drive, and Little Buffalo, All audible 1 visual alarms and
SCADA functioned when the wet well was allowed to fill up to the high well level or when
the high-water float was lifted. All generators started and operated under load when power
was turned off to the pump station. SCADA reported a power outage.
10. Three aerial lines were inspected. The aerial line off US 421 connected two elevated
manholes above a creek. There was no erosion along the base of the manholes and the
elevated aerial appeared straight. The aerial had no indication of rust. The aerial line on
Boone Trail is elevated above a dry creek bed. There are no piers and no bank erosion.
The third aerial line is a 12" DI with concrete side on both banks_ There was no indication
of erosion.
11. The Boone Trail right of way was inspected, The right of way was well maintained and free
of excess vegetation. There were no indication of depressions or sinkholes.

Inspecton Summary
