REGAN Secretary LINDA CULPEPPER Director Carlos Hernandez 6212 Old Kemp Road Durham, NC 27703 NORTH CAROLINA Environmental Quality May 6, 2019 Subject: Compliance Evaluation Inspection Single Family Wastewater Treatment System NPDES General Permit NC0550000 Certificate of Coverage NCG551657 Durham County Dear Mr. Hernandez: On May 1, 2019, Vanessa Manuel from the Raleigh Regional Office visited your single-family residence (SFR) wastewater treatment system to evaluate compliance with the subject NPDES General Permit. The assistance of your wife, Beatrez Hernandez, during the inspection was greatly appreciated. The inspector left a packet of information regarding Single Family Treatment Systems and the requirements of the General Permit at the residence. Our records indicate the treatment system consists of a septic tank with effluent filter, secondary recirculation/dosing tank, E-Z Treat treatment pod with UV disinfection, control panel with alarm, effluent discharge pipe with sample port, and outfall pipe with rip -rap erosion protection. General Permit NCG550000 and Certificate of Coverage (COC) NCG551657 authorize the discharge of domestic wastewater from your treatment system to receiving waters designated as an unnamed tributary to Rocky Branch (classified WS-IV; NSW) in the Neuse River Basin. The authorized discharge is in accordance with the effluent limits and monitoring requirements established within the General Permit. The checked boxes below show what conditions were noted at your facility: ® Treatment system operation: The wastewater treatment system shall be maintained at all times to prevent seepage of sewage to the surface of the ground. Once the residence is occupied and ilvastervater is being generated, please ensure the treatment system is properly operated and maintained to ensure optimal performance. ® Pumping the septic tank: You are required to inspect the septic tank at least yearly to determine if solids must be removed or if other maintenance is necessary. Septic tanks should be pumped out every five years or when the solids level is found to be more than 113 of the liquid depth in the septic tank compartment, whichever is greater. A pumping company can check the status periodically and determine when pumping is required. During the inspection, Mrs. Hernandez indicated the residence is currently under construction and is not currently occupied. After the home is occupied, please ensure the septic tank is checked periodically as required. DNorth Carolina Department of Environmental Quality ! New rules put into place on August 1, 2007 require all SFR systems to have a means of disinfection (and dechlorination when chlorine tablets are used to disinfect, if the system was installed since that date). A UV disinfection system provides effective disinfection and prevents/limits harmful bacteria from discharging to the environment. Unless the UV disinfection unit is monitored by telemetry, the unit shall be checked weekly. The lamp(s) and quartz sleeve(s) shall be cleaned or replaced as needed to ensure proper disinfection of the discharged wastewater. Once the residence is occupied and wastewater is being generated, please ensure the UV disinfection unit is properly operated and maintained to ensure optimal performance. ® Analyzing the effluent: fart 1. A., Effluent Limitations and 1 1onitoring Requirements, within General Permit NCG550000 requires a permittee to sample and analyze the effluent leaving his/her treatment system prior to discharge annually. Parameters to be sampled and analyzed include Flow, BOD (Biochemical Oxygen Demand), Total Suspended Solids, Fecal Coliform and Total Residual Chlorine. Once the residence is occupied and wastewater is being generated, please ensure a representative sample of the effluent is collected and analyzed at least once a year as required. N Discharge outlet location. The permittee is required to conduct a visual review of the outfall location at least twice each year (one at the time of sampling) to ensure that no visible solids or other obvious evidence of system malfunctioning is observed. Any visible signs of a malfunctioning system shall be documented and steps taken to correct the problem. The discharge pipe was visible and accessible the day of the inspection. Please continue to ensure the outlet is always maintained and cleared of vegetation, soil and leaves. Part II Section B.12 of General Permit NCG550000 requires the permittee to "pay the annual administering and compliance monitoring fee within thirty days after being billed by the Division." The annual fee ($60 per year) for the period March 1, 2019 through February 28, 2020, was due by March 9, 2019. This fee has not been paid and is overdue. Payment in the amount of $60 must be remitted to the Division as indicated on the attached Annual Permit Fee Invoice.
