REGAN Secretary LINDA CULPEPPER Director NORTH CAROLINA Environmental Quality February 5, 2019 Ms. Kimberly Kocik-Tholen and Mr. Allen Tholen and Email 5311 Flint Ridge Church Road Marshville, North Carolina 28103 Subject: Non -Discharge Permit Inspection Wastewater Surface Irrigation System Permit #WQ0003158 Union County Dear Mr. and Mrs. Tholen: Staff of the Mooresville Regional Office (MRO) conducted a compliance inspection on January 25tn 2019. The MRO would like to thank you both for your time during the inspection. The owner inspection logs were found to be in good order, at the time of inspection. The system appears well maintained and is operating as designed; however, the current design allows the system to irrigate under wet weather conditions which is not ideal and presently beyond owner control. Please note the following comments to maintain overall program compliance: • Wet weather conditions for the past several months have caused ponding conditions in the application field. Should you notice additional run-off related to your wastewater irrigation or detect odors, please contact Maria Schutte at the phone number listed below for assistance. • All tanks should be assessed annually for residuals and scum layers and pumped as needed. Per conversation, tanks have historically been pumped when site inspections are scheduled, but wet weather conditions currently prohibit access. Pumping will be scheduled when grounds are dry enough to support heavy equipment. In addition, you inquired about use of a local plumber should a pump fail. You may utilize a local plumber. I am also including contact information for three contractors who have serviced other treatment units in this region and may be of assistance with all components of your system should you need it. Page: 2 Permit: WQ0003158 Owner - Facility: Kimberly A Kocik Inspection Date: 01/25/2019 Inspection Type : Compliance Evaluation Reason for Visit: Routine Permit Status Yes No NA NE # Is the current resident in the home the Permittee? ❑ ❑ ❑ # If not, does the resident rent from the Permittee? ❑ ❑ ❑ Change of Ownership form needed? (Mail the form with the inspection letter) ❑ ❑ ❑ # Is there an inspection and maintenance agreement with a contractor? ❑ ❑ ❑ If YES, who is the contractor (include contact info)? Owner completed a change of ownership form with their previous renewal application, but first needs to add spouse to property deed. Comment: Owner(s) do a good job of maintaining the system themselves. Will contact local plumber for pump repairs, as needed. Septic Tank Yes No NA NE *** The septic tank and filters should be checked annually and pumped/cleaned as needed. Is all wastewater from the home connected to the septic tank? ❑ ❑ ❑ # Does the permittee/resident know where the septic tank is located? ❑ ❑ ❑ Has the septic tank been pumped in the last 5 years? ❑ ❑ ❑ If YES, describe if known and proof (include date pumped): The tank was pumped in 2016, prior to inspection. Owner would have pumped the tank prior to this inspection, but the past several months of wet weather has currently made their property inaccessible for heavy vehicles. # Does the septic tank have an EFFLUENT FILTER or SANITARY T? ❑ ❑ ❑ If FILTER, when was the filter cleaned and by who? NA Comment: This system design uses a sand filter. Sand Filter/Treatment Pods Yes No NA NE *** Accessible sand filter surfaces shall be raked/leveled every 6 months and vegetative growth shall be removed manually. *** # Is system something other than a sand filter? ❑ ❑ ❑ # If YES, what kind? (examples - Peat, Textile or brand name - Advantex, etc.) # Does the permittee know where the sandfilter is located? ❑ ❑ ❑ Does the sandfilter require maintenance? ❑ ❑ ❑ If maintenace is required, explain: Sand filter appears to be well maintained. Comment: Rainwater entering the sand filter may also trigger the system to irrigate. Disinfection UV Yes No NA NE *** UV unit shall be checked weekly. Lamps/sleeves should be cleaned/replaced as needed to ensure proper disinfection. *** Is UV working? ❑ ❑ ❑ Has the UV Unit been serviced and bulbs cleaned? ❑ ❑ ❑ Page: 3 Permit: WQ0003158 Owner - Facility: Kimberly A Kocik Inspection Date: 01/25/2019 Inspection Type : Compliance Evaluation Reason for Visit: Routine # Who completes the weekly check for the UV? ( Non -Discharge) NA Comment: Disinfection is acheived by chlorination. Disinfection Tablets Yes No NA NE *** Tablet chlorinator unit shall be checked weekly to ensure continuous and proper operation. *** Does the permittee have the correct chlorine tablets? (If none, mark No) ❑ ❑ ❑ # Does the Permittee know the location of the chlorinator? ❑ ❑ ❑ Were chlorine tablets observed in the chlorinator? ❑ ❑ ❑ Are tablets contacting water? (If possible, poke them to determine.) 0 ❑ ❑ ❑ Comment: Tablets are durrently purchased from McCall Brothers Well Drilling in Charlotte, NC. Pump Tank Yes No NA NE *** All pump and alarm sytems shall be inspected monthly. (Non -Discharge) *** Is the pump working? ❑ ❑ ❑ Is the audible and visual high water alarm operational? ❑ ❑ ❑ # Does the permittee know how to check the pump & high water alarm? 0 ❑ ❑ ❑ # Last functional test: During this site visit. Comment: The alarm was tested at the box. The float sensor was not pulled. Per previous inspection reports, this system was designed with only an audible alarm (there is no light fixture). Drip or Irrigation Yes No NA NE *** Irrigation sysetm shall be inspected monthly to ensure system is free of leaks and equipment is operating as designed. *** # Type of system (DRIP or IRRIGATION): Spray Irrigation # If IRRIGATION, number of sprinkler heads: 3 Are buffers and setbacks adequate? 0❑ ❑ ❑ Is the site free of ponding and runoff? ❑ ❑ ❑ Does the application equipment appear to be working properly? ❑ ❑ ❑ Is there a minimum two wire fence surrounding the entire irrigation area? 0 ❑ ❑ ❑ Comment: Several months of wet weather have created pondinq issues throughout the region. Most single family systems will irrigate when the float is activated by volume, regardless of field conditions. MRO staff is currently advising residents to monitor and contact the MRO should they notice run-off or odors. The perimeter of the land application site was walked during this inspection. There was significant pondinq noted in the field (even worse on the surrounding property), the spray system was running as tank volume triggered the pump(s). There were no odors or signs of run-off detected at the time of inspection. General # Are the treatment units locked and/or secured? # Has resident had any sewage problems? Yes No NA NE ■❑❑❑ ❑■❑❑ Page: 4 Permit: WQ0003158 Owner - Facility: Kimberly A Kocik Inspection Date: 01/25/2019 Inspection Type : Compliance Evaluation Reason for Visit: Routine If YES, explain: Units are secured by limited (private property) access and heavy concrete covers, but not locked. Does the system match the permit description? ❑ ❑ ❑ If NO, explain: Is the system compliant? ❑ ❑ ❑ Is the system failing? (If yes, take pictures if possible) ❑ ❑ ❑ If system is failing, describe any exposures to people/animals or environmental risks. Appears compliant, barring the uncontrolable weather related conditions noted previously. Comment: Owner mainatins aood records. inspection loa.
