4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The project study area is generally commercial in nature with Wake Stone Corporation, McLane Carolina, The Cheesecake Factory Bakery, Draka Elevator Products, Original Log
Cabin Homes and LS Tractor USA located in the project area. Immediately adjacent to EAt 145 interchange are several chain motels, fast food restaurants and gas station convenience
stores along with NCDOT rights -of -ways and impervious surfaces. Residential and agricultural areas are on the outside fringe of the commercial properties within the project study area.

4h. Explain the purpose of the proposed project:*
The purpose of the proposed project is to improve traffic operations at the 1-95 and NC 4 interchange (exit 145) and better accommodate truck traffic through the interchange by
correcting geometric deficiencies. 1-95 is a major regional freight corridor with about 17 percent heavy vehicles. Roadways in the project study area serve high volumes of trucks exiting
1-95 to access commercial and industrial development, as well as services such as gas, food, and lodging, near the interchange.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
The proposed project would replace the existing trumpet interchange at 1-95 and NC 4 with a new diamond interchange. The diamond interchange would include roundabouts at the
ramp terminals on NC 4. In addition, Halifax Road would be realigned to tie into the new interchange from the west, and the existing Halifax Road bridge over 1-95 would be removed. No
improvements would be made to the existing 1-95 through lanes. Traffic would be maintained on existing facilities for the duration of the construction. Nighttime lane closures on 1-95
would be used to construct the new NC 4 bridge over 1-95. Heavy road and bridge building equipment would be used.

E. Impact Justification and Mitigation
1. Avoidance and Minimization
1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
The preferred alternative was selected because it best fulfills the purpose and need for the project by providing improved traffic operations at the
interchange, corrects geometric deficiencies, and avoids impacts to Swift Creek located north of the existing interchange. The project includes only
one bridge over 1-95 with no improvements to the through lanes of 1-95.
1 b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
All construction equipment will be staged and operated from upland areas. NCDOT will also install silt fencing to protect streams and wetlands
adjacent to the project using design standards for Sensitive Watersheds (15ANCAC 04B.124). Safety fencing will be used to delineate jurisdictional
and environmentally sensitive areas from encroachment.

Project has no permeant jurisdictional wetland impacts. Also, culvert construction (30" WSP) in southeast quadrant of project is limited to 23 linear feet of permeant impacts to a
jurisdictional stream channels which falls below Regional conditions threshold of stream impacts / requirement for compensatory mitigation.

Red -cockaded Woodpecker (RCW) (Endangered) is listed as "Historic" for Nash County and therefore detail surveys are note required for this species
due to its historic status within Nash County. However, NCNHP records review indicates location (Burton 1979) of a cavity tree (nesting habitat) located
one mile south of the project study area. No actively nesting RCW habitat was documented by NCNHP. Habitat surveys conducted on July 23, 2018,
showed no nesting habitat in the project area and only potential marginal foraging habitat located in the northern end of the project study just outside
of the project study area. (No Affect). / Tar River Spinymussel (Endangered), Yellow Lance (Threatened), and Atlantic Pigteo (Proposed Threatened)
is included in the IPaC TE listed for project area dated April 10, 2019. Swift Creek which is located outside of the project area, but adjacent to the
project area has an "element occurrence " determination for these species by NCNHP data base for this area. US FWS consultation letter dated April
19, 2019, concurs with a finding of may affect, but not likely to adversely affect for these listed species associated with the new bridge not crossing
over the Swift Creek jurisdictional waters and NCDOT agreement for the project using design standards for Sensitive Watersheds (15ANCAC
04B.124). (MANLAA) // 1 Sumac (Endangered) is included in the IPaC TE list for the project area dated April 10, 2019. A presence/absence
survey was conducted on July 23, 2018, for this species. No Mici Sumac was found within the project study area during this field survey. A review
of NCNHP date base records indicates no known element occurrence for this species within 1.0 mile of the project study area. (No Affect)
