following reason:
Part A #7,8 - the coordinates listed do not match with the street address; they put you somewhere in Selma, NC.
Please verify the coordinates specific to your site.

Part D #5 - we just need the erosion and sediment control plan approval letter from the Town of Wake Forest.
The letter should have the information that you entered in Part D, #1 and #2.
