Inspection Summary:
This inspection was conducted to verify the Notice of Intent for coverage under General Permit NCG100000 by Igor Putin.
Upon inspection, it was found that the site be granted the certificate of coverage, however the associated map will need
revisions to reflect the concentrated outfall areas, adjacent waterways, wetlands, stockpiles, oil storage, and any other
locations potentially impacted by stormwater. Please submit the revisions to the outfall map within 30 days.

Comment: The map submitted with the permit application noted that all the stormwater runoff was sheet
flow. Upon inspection, we observed multiple areas where concentrated flow leaves the site. This
could Dotentially be listed as a desianated outfall.
