Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:* The existing stream is deeply incised and moving laterally toward the Gates backyard and home. Excessive erosion, sediment export, and property loss are occurring. The stream is in a moderately wooded neighborhood which was plated prior to the buffer rules. The riparian buffer is mowed with minimal vegetation supporting the streambank. The stream banks are high and unstable. 4b. Explain the purpose of the proposed project:* The project is intended to rebuild the unstable and actively eroding banks, to prevent further property loss, using vegetated geo-lifts and riparian vegetation. 41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:* This project will involve rebuilding approximately 451f of unstable streambank. The streambank will be rebuilt using vegetated geo-lifts, soil, and stone. The vegetated geo-lifts will be planted with live stakes and wetland plants. Grass will be planted on all other slopes and disturbed soil. The design was completed using natural channel design techniques. The work Wil be accomplished using a small excavator and hand tools. 4j. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project: The project is designed to improve streambank stability which will better protect riparian vegetation and property. 1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques: The project was design to limit impact to existing riparian vegetation which are protecting portions of streambanks on the property. No streambank vegetation will be removed to construct this project. The project will be completed using small machinery and reusing materials on-site to the maximum extent possible. 2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State 2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State? r Yes B No 2b. If this project DOES NOT require Compensatory Mitigation, explain why: The impacts are caused by stream restoration work and are temporary F. Stormwater Management and Diffuse Flow Plan (required by DWR) 1. Diffuse Flow Plan 1a. Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules? r Yes F No If no, explain why: No new impervious or discharge locations are being constructed. Diffuse flow will be maintained by matching the wasting grades on-site. 2. Stormwater Management Plan 2a. Is this a NCDOT project subject to compliance with NCDOT's Individual NPDES permit NCS000250? r Yes 6 No 2b. Does this project meet the requirements for low density projects as defined in 15A NCAC 02H .1003(2)? 6 Yes t^ No Comments: No new impervious or discharge locations are being constructed. Diffuse flowwill be maintained by matching the existing grades on-site. This project is a stream restoration project and involves stabilization of streambanks G. Supplementary Information 1.
