The information uploaded did not include a letter of approval from DEQfor Project #Pitt-2019-017 but was instead the
E&SC Plan. Also, the Index number entered for 15.b was "NEU, TAR" which are River Basin abbreviations -- not index
numbers, and the entry noted two rivers. Please enter each *separately* (indicate stormwater will flow to additional
waterbodies to do this). Please resubmit an NOI with these items addressed. For questions, please contact Bethany
